轻、中度隐匿性阴茎治疗探讨——附75例报告

目的通过随访回顾75例轻中度隐匿性阴茎病例,了解手术治疗与保守治疗的差异性。方法方便选取该院自2006年1月—2015年6月收治的轻中度隐匿性阴茎患者。手术组46例,保守组29例,对两组的住院天数、医疗费用、阴茎显露满意度进行比较。结果随访36～72个月(青春期后),平均随访时间41.2月。手术组阴茎显露满意度89.1%,保守组阴茎显露满意率86.2%,对比差异无统计学意义(χ~2=0.144,P=0.704);手术组平均治疗费用(8 869.6±326.5)元,保守组平均治疗费用(768.6±103.5)元,对比差异有统计学意义(t=156.286,P=0.000)。手术组平均住院时间(5.2±0.8)d,保守组平均住院时间(0.8±0.2)d,两组对比有明显差异(t=35.581,P=0.000)。结论轻中度的隐匿性阴茎可随年龄增长而于青春期改善或痊愈,手术治疗与保守治疗于青春期后其阴茎显露满意度无明显差异。青春期前可行保守治疗扩张包皮口或仅行包皮背侧切开术,减少患儿的手术痛苦及手术费用。

Objective To know the difference between the operative treatment and conservative treatment by reviewing 75 cases of patients with mild and moderate concealed penis.Methods The patients with mild and moderate concealed penis admitted and treated in our hospital from January 2006 to June 2015 were convenient selected including 46 cases in the operative group and 29 cases in the conservative group,and the length of stay,medical cost,and satisfactory degree of penis exposure were compared between the two groups.Results After follow-up for 36 to 72 months(after the puberty),and the average follow-up time was 41.2 months,and the satisfactory degree of penis exposure in the operation group and in the conservative group was respectively 89.1%and 86.2%,and the difference was statistically significant(χ^2=0.144,P=0.704),and the average treatment cost in the operation group and in the conservative group was respectively(8869.6±326.5)yuan and(768.6±103.5)yuan,and the difference was obvious(t=156.286,P=0.000),and the average length of stay in the operation group and in the conservative group was respectively(5.2±0.8)d and(0.8±0.2)d,and the difference was obvious(t=35.581,P=0.000).Conclusion With the increase of the age,the mild and moderate concealed penis can be improved or cured during the puberty,and there was no obvious difference in the satisfactory degree of penis exposure after the puberty between the operative therapy and conservative therapy,and we can only use the conservative therapy for expanding the ostium praeputiale or only dorsal prepucotomy to reduce the operative pains and cost of children.
