应用多种功能磁共振成像对胶质瘤进行评估价值探究 被引量:2

摘要 目的探讨应用多种功能磁共振成像对胶质瘤进行评估的价值,从而使脑胶质瘤能够得到精确的术前诊断。方法方便选取2016年1月—2018年1月该院收治的胶质瘤患者40例,患者的病情均经病理学检查确诊,为患者开展常规核磁共振成像(MRI)和功能MRI检查,统计分析本组患者MRI检查中的体素内不相干运动扩散加权成像(IVIMDWI)中的慢扩散系数、快速扩散系数、分布扩散系数以及灌注分数值。结果不同级别的胶质瘤中,rCBF值存在着较大的分布跨度,同时范围存在交叉性。三维增强梯度回波T2血管成像序列结果无法对胶质瘤的高低级别进行有效区分。高级别胶质瘤组患者病灶侧的慢扩散系数、快速扩散系数、分布扩散系数和灌注分数值灌注分数值分别为(0.73±0.06)×10^(-3)mm^2/s、(0.08±0.03)mm^2/s、(1.30±0.35)×10^(-3)mm^2/s、(0.39±0.08)f,对侧正常脑白质慢扩散系数、快速扩散系数、分布扩散系数和灌注分数值灌注分数值分别为(0.15±0.04)×10^(-3)mm^2/s、(0.02±0.01)mm^2/s、(0.45±0.11)×10^(-3)mm^2/s、(0.53±0.06)f。相较于对侧正常脑白质,高级别胶质瘤组患者的病灶侧的慢扩散系数、快速扩散系数和分布扩散系数更高,灌注分数值更低,均差异有统计学意义(t=28.408、6.164、7.499、5.062,P<0.05)。结论在对脑胶质瘤开展诊断时,通过应用多种磁共振功能成像序列,可对肿瘤存在的异质性进行有效避免,使患者的疾病诊断准确率得到有效提高。 Objective To study the value of multiple functional magnetic resonance imaging in evaluating the glioma thus obtaining the preoperative diagnosis of glioma.Methods 40 cases of patients with glioma admitted and treated in our hospital from January 2016 to January 2018 were convenient selected and confirmed by the pathology,and the patients were for routine MRI and functional MRI,and the slow diffusion coefficient,rapid diffusion coefficient,distribution diffusivity and perfusion fraction in the IVIM-DWI were statistically analyzed.Results There was a bigger distribution span in the rCBF value,and the range was crossed,and the three dimensional enhanced gradient echo T2 angiography sequence results failed to effectively distinguish the level of glioma,and the slow diffusion coefficient,rapid diffusion coefficient,distribution diffusivity and numerical value of perfusion fraction were respectively(0.73±0.06)×10^-3mm^2/s,(0.08±0.03)mm^2/s,(1.30±0.35)×10^-3mm^2/s,(0.39±0.08)f at the focal side in the high-grade gliomas group,and the slow diffusion coefficient,rapid diffusion coefficient,distribution diffusion coefficient and perfusion fraction of normal white matter at the opposite site were respectively(0.15±0.04)×10^-3mm^2/s,(0.02±0.01)mm^2/s,(0.45±0.11)×10^-3mm^2/s,(0.53±0.06)f,and the slow diffusion coefficient,rapid diffusion coefficient and distribution diffusion coefficient at the focal side in the high-grade gliomas group were higher than that of the contralateral normal white matter,and the differences were statistically significant(t=28.408,6.164,7.499,5.062,P﹤0.05).Conclusion The application of multiple functional magnetic resonance imaging sequences in diagnosis of gliomas can effectively avoid the heterogenicity of tumors,thus effectively improving the disease diagnosis accuracy rate.
