预注氟比洛芬酯对无痛人工流产患者丙泊酚麻醉效果影响的研究

Study on the effect of pre-injection flurbiprofen axetilon the anesthetic effect of propofolin patients with painless artificial abortion

目的分析预注氟比洛芬酯对无痛人工流产手术患者实施丙泊酚麻醉的影响效果。方法将2017年6月～2018年6月我科收治的80例行无痛人工流产手术的患者按随机数字表法分为两组,对照组行舒芬太尼复合丙泊酚静脉麻醉,观察组在麻醉诱导前静脉注射氟比洛芬酯后行丙泊酚静脉麻醉。监测入室后、麻醉诱导后、术中以及术毕时的平均动脉压和心率。记录两组丙泊酚的用量,麻醉恢复时间,VAS评分及不良反应等情况。结果观察组在术毕(T3)时的MAP、HR明显低于对照组,差异有统计学意义(P <0.05);观察组术中丙泊酚追加量和总用量,明显小于对照组,差异有统计学意义(P <0.05);观察组术中呼吸抑制、静注丙泊酚诉疼痛的比例明显小于对照组,差异有统计学意义(P <0.05);两组在消化道反应及头晕、头痛等不良反应的发生率比较,差异无统计学意义(P> 0.05);两组在VAS评分及麻醉恢复时间比较,差异无统计学意义(P> 0.05)。结论预注氟比洛芬酯能有效减少丙泊酚的用量,维持患者的血流动力学稳定,提高麻醉的效果,不影响麻醉的恢复,不良反应少,安全性高,值得推广。

Objective To analyze the effect of pre-injection flurbiprofenaxetil on the anesthetic effect of propofol in patients with painless artificial abortion.Methods 80 patients who were admitted and underwent painless artificial abortionin our department from June 2017 to June 2018 were selected and they were divided into two groups according to random number table method.The control group was given intravenous anesthesia with sufentanil combined with propofol while the observation group was given intravenous anesthesia with flurbiprofen axetilbefore anesthesia induction,followed by intravenous anesthesia withpropofol.The mean arterial pressure and heart rate after entering the room,after anesthesia induction,during the operation and after the operation were monitored.The dosage of propofol,anesthetic recovery time,VAS score and adverse reactions in the two groups were recorded.Results MAP and HR after surgery(T3)in the observation group were significantly lower than those in the control group(P<0.05).The intraoperative additional and total dosage of propofolin the observation group was significantly less than that in the control group(P<0.05).The proportions of intraoperative respiratory depression and complaint ofpain of intravenous injection of propofol in the observation group were significantly lower thanthose in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of gastrointestinal reaction,dizziness,headache and other adverse reactions between the two groups(P >0.05).There was no statistically significant difference in VAS score and anesthesia recovery time between the two groups(P >0.05).Conclusion Pre-injection flurbiprofenaxetilcan effectively reduce dosage of propofol,maintain the hemodynamic stability of patients and improve the effect of anesthesia.It does not affect the recovery of anesthesia and it hasfew adverse reactions and high safety,which is worthy of promotion.
