“医养融合”模式下社区老年护理效果评价

Evaluation of the effect of community geriatric nursing under the mode of “integration of medical service and aged service”

目的:探索团队护士实施的老年护理在"医养融合"模式中的作用。方法:2017年7月—2018年6月对延吉社区卫生服务中心辖区内2家养老院内228例老人按日常生活能力评分(ADL)将老人分为A(<60分,54名)、B(> 60分,但<80分,111名)、C(≥80分,45名)三类进行分级管理,由社区护士进行日常生活能力指导干预,并对养老院护理员进行介护培训。12个月后,采用生活质量(QLI)评分表对54名失能老人进行生活质量评估;采用生活条件满意度表格对200名老人进行生活条件满意度指数测评。结果:干预前,54名失能老人的QLI为(4.16±1.08)分,干预后为(6.18±1.43)分,差异有统计学意义(P <0.01)。生活条件满意度测评的平均分值从干预前的(1.74±0.43)分上升至干预后的(3.30±0.29)分,差异有统计学意义(P <0.01)。结论:"医养融合"模式下的团队护士的老年护理服务对促进失能老人的日常生活能力有效,满足了老人多样化的养老需求,且对老人的生活满意度也有所提升,值得社区推广。

Objective: To explore the role of elderly nursing implemented by team nurses in the mode of integration of medical service and aged service. Methods : From July 2017 to June 2018, 228 elderly people in two nursing homes under the jurisdiction of the Yanji Community Health Service Center were divided into A (<60 points, 54 cases), B ( >60 points, but <80 points, 111 cases) and C (≥80 points, 45 cases) according to the daily living ability score for three levels of management, and the community nurses conducted daily life ability guidance interventions and provided nursing training for nursing home caregivers. After 12 months, the quality of life of 54 disabled elderly people was assessed by quality of life index (QLI) score;the life condition satisfaction index was evaluated by using the life condition satisfaction table for 200 old people. Results: Before intervention, the QLI of 54 disabled seniors was (4.16±1.08), after intervention that was (6.18±1.43), and the difference was statistically significant ( P < 0.01). The average score of life condition satisfaction evaluation increased from (1.74±0.43) points before intervention to (3.30±0.29) points after intervention, and the difference was statistically significant ( P < 0.01). Conclusion: The elderly nursing service of team nurses under the mode of "integration of medical service and aged service" is effective in promoting the daily living ability of disabled elderly people, satisfies the diversified needs of the elderly for the aged service, and the life satisfaction of the elderly has been also improved, which is worthy of community promotion.
