可注射骨水泥椎弓根螺钉治疗骨质疏松性椎体压缩骨折的临床疗效观察

Clinical observation of pedicle screw injected bone cement in the treatment of osteoporotic vertebral compression fractures

目的:观察可注射骨水泥椎弓根螺钉治疗骨质疏松性椎体压缩骨折的疗效。方法:回顾性分析采用可注射骨水泥椎弓根螺钉治疗骨质疏松性椎体压缩骨折患者40例,于术前及术后1个月、3个月、6个月和12个月时进行随访,记录患者的VAS评分、ODI指数、Cobb角、椎体前缘百分比以及末次随访时进行Macnab疗效评价。结果:与术前比较,术后1个月患者的VAS评分和ODI指数均显著下降,差异有统计学意义(P <0. 05);随着时间时间延长,患者VAS评分和ODI指数进一步显著降低,差异有统计学意义(P <0. 05)。与术前比较,术后1个月患者的Cobb角和椎体前缘百分比均显著下降,差异有统计学意义(P <0. 05);术后3个月时患者的Cobb角显著优于术后1个月。末次随访时,治疗优良率达85%。结论:可注射骨水泥椎弓根螺钉可有效改善骨质疏松性椎体压缩骨折患者的疼痛症状、脊柱障碍、Cobb角和椎体前缘百分比,是一种治疗骨质疏松性椎体压缩骨折的有效方法,但仍需进一步大样本和远期研究。

Objective To observe the effect of pedicle screw injected with bone cement in the treatment of osteoporotic vertebral compression fractures.Method A retrospective analysis of 40 cases of osteoporotic vertebral compression fractures treated by pedicle screw injected with bone cement in our hospital.The patients were followed up before and 1 months,3 months,6 months,and 12 months after the operation.The patients’VAS score,ODI index,Cobb angle,the percentage of anterior vertebral edge were recorded and the evaluation of Macnab were evaluated at the last follow-up.Results Compared with preoperative,1 month after operation,the VAS score and ODI index decreased significantly,the differece was statistically significant(P<0.05).As time went on,the VAS score and ODI index decreased significantly,the differece was statistically significant(P<0.05).Compared with preoperative,1 month after operation,the Cobb angle and the percentage of anterior vertebral body decreased significantly,the differece was statistically significant(P<0.05).The Cobb angle of patients at 3 months after operation was significantly better than that of 1 month after operation.The good rate of treatment was 85%at the last follow-up.Conclusion Pedicle screw injected with bone cement can effectively improve the pain symptoms,spinal disorders,Cobb angle and the percentage of anterior vertebrae in osteoporotic vertebral compression fractures.It is an effective method for the treatment of osteoporotic vertebral compression fractures.However,further large and long-term studies are still needed.
