米非司酮治疗早期异位妊娠的有效性

Effect of Mifepristone on Early Ectopic Pregnancy

目的分析米非司酮治疗早期异位妊娠的有效性及对患者生育能力的影响。方法选取100例早期异位妊娠患者作为观察对象,根据患者意愿分为观察组与对照组,各50例。对照组单用米非司酮治疗,观察组采用米非司酮联合甲氨蝶呤治疗。比较2组患者的异位妊娠终止效果,比较2组患者包块消失时间以及住院时间,记录2组治疗期间的不良反应,随访1. 5 a观察2组患者的妊娠率以及妊娠结局。结果 2组患者异位妊娠终止成功率(80. 0%vs 78. 0%),差异无统计学意义(P> 0. 05);但观察组包块消失时间(25. 8±6. 5) d以及住院时间(12. 5±5. 4) d,均短于对照组(31. 5±6. 9) d和(20. 6±6. 8) d,差异有统计学意义(P <0. 05); 2组患者治疗期间均无明显不良反应发生;随访1. 5 a发现,2组患者自然妊娠率(24. 0%vs 12. 0%)与妊娠结局差异无统计学意义(P> 0. 05)。结论米非司酮在早期异位妊娠中具有较好的临床疗效,对妊娠结局无明显影响,与甲氨蝶呤联合应用时可缩短包块消失时间和住院时间。

Objective To analyze the effectiveness of mifepristone in the treatment of early ectopic pregnancy and its influence on the fertility of patients.Methods 100 cases of early ectopic pregnancy were selected as the observation objects.According to the wishes of the patients,they were divided into observation group and control group,50 cases in each group.The control group was treated with mifepristone alone,while the observation group was treated with mifepristone combined with methotrexate.Termination effect of ectopic pregnancy,mass disappearance and hospitalization time were compared,and the adverse reactions were recorded during the treatment in the two groups.For one and a half year follow-up observation of two groups of patients the pregnancy rate and pregnancy outcome were observed.Results The two groups of patients with ectopic pregnancy success rate difference was not statistically significant 80.0%vs 78.0%;but for the observation group,the mass disappeared time(25.8±6.58)and hospitalization time(12.5±5.4)d were significantly less than the control group(31.5±6.9)d/(20.6±6.8)d(P<0.05);There was no obvious reactions in two groups of patients during treatment;after one and a half year follow-up,the two groups of patients with spontaneous pregnancy rate and pregnancy outcome without significant difference.Conclusion Mifepristone in early ectopic pregnancy has good clinical curative effect,and can improve the pregnancy rate of patients with no obvious effect on the outcome of pregnancy,can shorten the mass disappear time and hospitalization time in combination with methotrexate is worthy of clinical application.
