经尿道前列腺等离子切割术治疗良性前列腺增生对性功能的影响研究

Effect of transurethral plasmakinetic resection of prostate on the sexual function of patients with benign prostatic hyperplasia

目的探讨经尿道前列腺等离子切割术治疗良性前列腺增生对性功能的影响。方法选择天门市第一人民医院2016年5月至2017年5月诊治的良性前列腺增生患者94例,按照随机表法分为观察组47例与对照组47例。观察组采用经尿道前列腺等离子切割术治疗,对照组采用开放前列腺切除术。结果观察组置管时间、膀胱冲洗时间及住院时间短于对照组,术中出血量少于对照组,差异具有统计学意义(P<0.05)。两组术后3个月IPSS评分和QOL评分降低,观察组:t=36.8635、16.8089,对照组:t=26.3074、12.1431,差异具有统计学意义(P<0.05);观察组术后3个月IPSS评分和QOL评分低于对照组,t=12.0801、9.1616,差异具有统计学意义(P<0.05)。两组术后3个月最大尿流率增加而残余尿量降低,观察组:t=20.3133、61.1535,对照组:t=16.6971、45.4792,,差异具有统计学意义(P<0.05);观察组术后3个月最大尿流率高于对照组而残余尿量低于对照组,t=5.9473、18.9685,差异具有统计学意义(P<0.05)。两组术后3个月IIEF-5评分降低,观察组:t=5.8736,对照组:t=11.2418,差异具有统计学意义(P<0.05);观察组术后3个月IIEF-5评分高于对照组,t=6.6070,差异具有统计学意义(P<0.05)。结论经尿道前列腺等离子切割术治疗良性前列腺增生效果显著,对患者性功能影响小,具有重要研究价值。

Objective To Investigate the effect of transurethral plasmakinetic resection of prostate on the sexual function of patients with benign prostatic hyperplasia(BPH).Methods 94 patients with BPHin our hospital from May 2016 to May 2017 were selected and randomly divided into observation group(47 cases)and control group(47 cases).The observation group was treated with transurethral plasmakinetic resection of prostate,while the control group was treated with open prostatectomy.Results The time of catheterization,the time of bladder irrigation and the length of hospital stay in the observation group were all shorter than those in the control group,and the amount of intraoperative blood loss was less in the observation group than that in the control group(P<0.05).After3 monthsafter surgery,theIPSS score and QOL score decreased in the two groups(observation group:t=36.8635,16.8089,the control group:t=26.3074,12.1431,P<0.05);the IPSS score and QOL score in theobservation group at 3 months after surgery were lower than those in the control group(t=12.0801,9.1616,P<0.05).The Qmax increased and residual urine volume decreased in both groups at 3 months after surgery(the observation group:t=20.3133,61.1535,control group:t=16.6971,45.4792,P<0.05)and the Qmaxin the observation group was higher than that in the control group and the residual urine volume was lower than that in the control group(t=5.9473,18.9685,P<0.05).The IIEF-5 score of the two groups decreased at 3 months after surgery(observation group:t=5.8736,control group:t=11.2418,P<0.05)and the score of IIEF-5 in the observation group was higher than that in the control group(t=6.6070,P<0.05).Conclusions Transurethral plasmakinetic resection of prostate is effective in the treatment of benign prostatic hyperplasia,with little influence on the sexual function of patients.
