经尿道前列腺电切术联合内分泌去势疗法治疗晚期前列腺癌临床疗效

Clinical efficacy and literature review by transurethral resection of prostate combined with endocrine castration therapy advanced prostate cancer

目的观察晚期前列腺癌合并尿潴留的患者在行经尿道前列腺电切术(TURP)并结合内分泌去势疗法的临床疗效。方法选取2008年3月至2012年3月的30例晚期前列腺癌合并尿潴留的患者,其中采取TURP结合内分泌去势治疗的患者13例,单纯行内分泌治疗的患者17例。观察两组患者治疗前后的血清前列腺特异抗原(PSA)、最大尿流率、剩余尿量、国际前列腺症状评分(IPSS)以及5年生存率。结果两组患者血清PSA水平、剩余尿量、IPSS评分与最大尿流率等指标在治疗前比较差异无统计学意义(P> 0. 05)。但是在治疗后比较,两组患者上述指标水平对比,差异具有统计学意义(P <0. 01),而TURP+内分泌组的5年生存率、血清PSA与内分泌组相比差异无统计学意义(P> 0. 05)。结论用TURP+内分泌疗法治疗晚期前列腺癌能够明显缓解患者的尿潴留症状,在不影响患者生存率的前提下能显著提高患者的生活质量,疗效很确切,安全性可靠。

Objective To observe the clinical efficacy of transurethral resection of prostate(TURP)combined with endocrine castration therapy for patients with advanced prostate cancer and urinary retention.Methods A total of 30 patients with advanced prostate cancer and urinary retention were selected,including 13 cases receiving TURP joint with endocrine castration therapy and17 cases receiving simple endocrine castration therapy.The serum prostate specific antigen(PSA),maximum flow rate,residual urine volume,International Prostate Symptom Score(IPSS)and the 5-year survival rate in the two groups were observed before and after treatment.Results There was no statistically significant difference in serum PSA level,residual urine volume,maximum urine flow rate and IPSS score between the two groups before treatment(P>0.05),while differences in the above indicators between the two groups after treatment were statistically significant(P<0.01)and difference in the 5-year survival rate and serum PSA between the two groups was not statistically significant(P>0.05).Conclusions TURP combined endocrine castration therapy can alleviate the urinary retention symptoms and improve the quality of life of patients with advanced prostate cancer,with exact,safe and reliable curative effect.
