2011—2017年辽宁省辽阳市某二甲医院用血情况分析

Analysis of Clinical Blood Use in a Second Grade A Comprehensive Hospital in Liaoyang City,Liaoning Province from 2011 to 2017

目的了解辽阳市某三甲医院临床用血情况,为制定血库用血计划及科学合理用血提供依据。方法回顾性分析辽阳市某三甲医院血库2011—2017年瑞美血库管理系统中临床用血患者信息及2015—2017年检验科瑞美检验网络系统中ABO血型患者信息。结果临床用血总量呈下降趋势,红细胞用量呈上升趋势,血浆用量呈下降趋势,其中,内科红细胞用量增加,外科红细胞用量减少,出院患者人均红细胞用量呈下降趋势,红细胞使用患者平均年龄>60.7岁,且患者年龄呈上升趋势。2015—2017年出库红细胞血型构成比为O>A>B>AB,检验科患者ABO血型构成比特征是B>O>A>AB。结论医院用血以红细胞和血浆制品为主,红细胞用量增加明显,外科用血量减少,内科用血量增加,老年人红细胞用量增加,根据医院用血特点,参照出库红细胞血型分布特点差异,制定临床用血计划和血库库存量,指导临床科学合理用血。

Objective To understand the situation of blood use in the hospital,and to provide the basis for the formulation of the blood bank,blood plan and scientific and rational use of blood. Methods Through utilizing a retrospective method,the clinical data of the blood usage was analyzed, which included the information of patients in the Ruimei Blood Bank Management System(2011- 2017)and the data of the group with ABO blood types in the Blood Test System(2015-2017). Results Decreases could be seen in the total consumption of blood and plasma,while the consumption of erythrocyte experienced an overall growth. Specifically,the usage amount of erythrocyte in physician increased. On the contrary,there was a trend of decline in the surgeon,and the per capita erythrocyte consumption of discharged patients. The average age of erythrocyte users was higher than 60.7,and the age maintained an upward trend. The consumption ranking of different outbound erythrocyte-blood-types was O>A>B>AB from 2015 to 2017, meanwhile, the blood types structure among the patients in the clinical laboratory was B>O>A>AB. Conclusion The consumption of erythrocyte and plasma products dominates the blood usage in the hospital,the amount of erythrocyte use has a rapid increase. Notaly,the consumption of blood in the surgeon declines,but,there is a growth in Surgery Department. The use of erythrocyte in the aged group is increasing. According to the characteristics of blood consumption and the constituent ratio difference in erythrocyte-blood-types in the hospital,planning on the blood for clinical use and stock management are formulated,which aims at creating a more adequate blood management system for clinical use.
