中药汤剂对小儿紫癜性肾炎阴虚血热型外周血Th17细胞的影响

Effect of Chinese herbal decoction on peripheral blood Th17 cells of children with anaphylactoid purpura nephritis of Yin-deficiency and blood-heat type

目的观察中药汤剂对小儿紫癜性肾炎(HSPN)阴虚血热型的治疗效果,探讨其对患儿外周血Th17细胞改变的影响。方法对2015年1月至2018年1月中山市中医院儿科门诊及住院部收治的HSPN病例进行收集,将确诊的100例HSPN患儿随机分为中药治疗组和西药治疗组各50例。中药治疗组治法为口服自拟中药汤剂紫癜1号方,西药治疗组治法为西医常规治疗,4周后评价疗效及观察外周血Th17细胞表达率。结果经治疗后,与西药治疗组临床总有效率58.0%(29/50)相比,中药治疗组高达82.0%(41/50),明显增高,差异有统计学意义(P<0.05)。在治疗后所有患儿外周血Th17细胞表达比率均出现了明显的差异,均下降,且中药治疗组的下降值更多,差异有统计学意义(P<0.05)。结论对HSPN的中西医治疗比较中,内服中药汤剂的效果更好,并且能降低患儿Th17细胞表达,能够调节患儿的免疫功能。

Objective To observe the therapeutic effect of traditional Chinese medicine decoction on children with purpura nephritis of Yin-deficiency and blood-heat type,and to explore its effect on peripheral blood Th17 cells in children with purpura nephritis.Methods The typical cases of Henoch-Sch nlein purpura nephritis were collected and sorted out,and 100 cases of children were eventually identified as the research subjects.They were randomly divided into the observation group(50 cases)and the control group(50 cases).The observation group was treated with traditional Chinese medicine decoction orally,while the control group was treated with routine western medicine.After 4 weeks,the curative effect was evaluated and the changes of Th17 in serum were observed.Results The clinical total effective rate of the observation group was 82.0%(41/50),significantly higher than that of control group(58.0%,29/50),the difference being statistical significance(P<0.05).There was no statistical difference in the expression of peripheral Th17 cells between the two groups before treatment(P>0.05);however,after treatment it was lower,and it was even lower in observation group than in control group(P<0.05).Conclusion In the course of treatment of Henoch-Sch nlein purpura nephritis,oral administration of traditional Chinese medicine decoction has better effect,which can reduce Th17 cells in children,and can regulate the immune function of children with Henoch-Sch nlein purpura nephritis.
