摘要
目的探讨MR扩散加权成像(DWI)单指数模型和体素内不相干运动(IVIM)模型参数在前列腺癌诊断中的价值。方法选取2016年1月至2018年6月在我院治疗的前列腺癌患者55例,共计72个病灶,同时选取前列腺增生患者50例,共计66个病灶作为对照,均给予DWI扫描,测量单指数模型参数[表观弥散系数(ADC)]以及IVIM模型参数[慢速扩散系数(D)、快速扩散系数(D*)、快速扩散所占容积分数(f)]。结果前列腺癌ADC值为(0.723±0.122)×10-3mm^2/s,明显低于前列腺增生,差异比较有统计学意义(P<0.05);前列腺癌D值为(0.702±0.101)×10-3mm^2/s,明显低于前列腺增生,差异比较有统计学意义(P>0.05);前列腺癌和前列腺增生D*和f比较差异无统计学意义(P>0.05);ADC和D诊断前列腺癌的ROC曲线下面积分别为0.800和0.854,P<0.05,截断值分别为0.811×10-3mm^2/s和0.802×10-3mm^2/s,灵敏度分别为82.00%和85.50%,特异度分别为75.00%和83.00%。结论 DWI单指数模型参数ADC、IVIM模型参数D在前列腺癌诊断中有较好的应用价值,值得临床使用。
Objective To investigate the diagnostic value of MR diffusion weighted imaging(DWI)single index model and incoherent voxel intra movement(IVIM)model parameters in prostate cancer.Methods A total of 55 cases of prostate cancer in our hospital from January 2016 to June 2018 were selected,with a total of 72 lesions,and 50 cases of prostatic hyperplasia were selected,with a total of 66 lesions were taken as the control,all of them were given DWI scan,the parameters of the single exponential model[apparent diffusion coefficient(ADC)]and IVIM model parameters[slow diffusion coefficient(D),fast diffusion coefficient(D*),and volume fraction(f)of rapid diffusion]were measured.Results The ADC value of prostate cancer was(0.723±0.122)×10^-3 mm^2/s,which was significantly lower than that of prostatic hyperplasia,and the difference was statistically significant(P<0.05).the D value of prostate cancer was(0.702±0.101)×10^-3 mm^2/s,which was significantly lower than that of prostatic hyperplasia,and the difference was statistically significant(P >0.05).There were no significant difference in D*and f between prostate cancer and prostatic hyperplasia(P >0.05).The area of ROC of ADC and D in the diagnosis of prostate cancer were 0.800 and 0.854,P<0.05,the cut-off values were0.811×10^-3 mm^2/s and 0.802×10^-3 mm^2/s,the sensitivity were 82.00%and 85.50%,and the specificity were 75.00%and 83.00%.Conclusion DWI single exponential model parameters ADC and IVIM model parameters D have a good application value in the diagnosis of prostate cancer,which is worthy of clinical application.
