水通道蛋白在帕金森患者不同大脑部位表达的磁共振影像研究

摘要 目的探究水通道蛋白在帕金森患者磁共振影像中不同大脑部位的表达及意义。方法前瞻性收集2015年1月到2015年12月在我院接受治疗的由神经内科锥体外系疾病专家确诊的PD患者34例,应用磁共振扩散加权成像(MR-DWI)多b值水通道蛋白分子成像技术测量不同脑区水通道蛋白表观扩散系数(Aquaporinapparent diffusioncoefficient,AQP-ADC)值;另外选取性别年龄相匹配的健康体检者24名作为对照组。应用Hoehn-Yahr改良分级对PD患者评级,然后比较轻度、中、重度PD组与对照组不同脑区的AQP-ADC值,并对不同脑区的AQP-ADC值与Hoehn.Yahr分级进行相关性分析。结果两组患者双侧半球广泛脑区的灰质体积数据比较显示,相比HC组,PD组显示双侧半球广泛脑区的灰质体积缺失,两侧半球脑体积缺失不对称,PD组与HC组间脑灰质体积有统计学差异(P<0.05);数据比较发现轻度PD患者红核、苍白球的AQP-ADC值明显高于对照组,数据差异具有统计学意义(P<0.05),而中重度PD患者较轻度PD患者明显降低,数据差异具有统计学意义(P<0.05);PD组红核、苍白球AQP-ADC值与Hoehn-Yahr分级成负相关(红核r=-0.478,苍白球r=-0.394,均P<0.05)。结论水通道蛋白对于研究帕金森患者不同大脑部位表达有其重要意义,为PD等神经退行性疾病的发病机制提供了新的观点和思路。 Objective To explore the expression and significance of water channel proteins in different parts of the brain in Parkinson’s patients with magnetic resonance imaging.Methods Prospective collection of 34 patients which diagnosed of PD in our hospital from January 2015 to December 2015 in our hospital for treatment of patients diagnosed by the Department of Neurology cone disease.The application of magnetic resonance diffusion weighted imaging(MR-DWI)b value of the apparent diffusion coefficient of water channel in different regions of the brain aquaporin protein molecular imaging measurement(Aquaporin apparent diffusion coefficient,AQP-ADC),in addition to selected age and gender matched,24 healthy subjects were enrolled as control group.The modified Yahr grading of PD patients by Hoehn and then compare the ratings,mild,moderate and severe PD group and control group in different brain regions of AQP-ADC,and the different brain regions of AQP-ADC values were analyzed with Hoehn.Yahr classification.Results Comparison shows that gray matter volume data of two groups of patients with extensive cerebral hemisphere,compared with HC group,PD group showed a loss of gray matter volume in widespread brain regions in both hemispheres,both sides of brain hemisphere volume loss of asymmetry,PD group and HC groups there were significant differences in gray matter volume(P<0.05).Comparison of data found that patients with mild PD red nucleus,globus pallidus AQP-ADC value was significantly higher than the control group,with significant difference(P<0.05),and the data of patients with PD was significantly lower in patients with mild PD,data statistically significant difference(P<0.05),group PD,red nucleus,Globus pallidus AQP-ADC negative correlation with HoehnYahr grade(red nucleus r=-0.478,globus r=-0.394,P<0.05).Conclusion Water channel protein has important significance for the study of the expression of different parts of the brain of Parkinson,and provides a new perspective and ideas for the pathogenesis of PD and other neurodegenerative diseases.
