摘要
目的探讨二维及彩色多普勒超声(CDFI)对甲状腺癌的诊断价值。方法选取2015年8月—2018年2月在我院采用二维超声和彩色多普勒血流显像检查并经手术和病理证实的58例甲状腺癌和47例甲状腺腺瘤患者进行研究。应用二维及彩色多普勒血流显像进行检查,分析患者不同超声表现,对病变内部和周边的血流分布情况[收缩期峰值流速(Vmax)及阻力指数(RI)]进行记录分析。结果本研究中超声诊断符合率为86.21%,甲状腺癌的二维声像图多表现为周边无声晕、低回声、微小钙化灶,检出率分别为87.93%,70.69%和63.79%;甲状腺癌内部血流信号的表现以Ⅰ、Ⅱ级为主,以血流丰富、高流速、高阻力为特征。2组在周边无声晕、低回声和微小钙化灶等二维声像图方面以及CDFI血流分级方面表现差异均显著(P<0.05),甲状腺癌组患者的Vmax、RI值均明显高于甲状腺腺瘤组,差异有统计学意义(P<0.05)。结论应用二维及彩色多普勒超声对甲状腺癌及甲状腺腺瘤病变部位的形态、内部回声类型和血流特点进行分析,对甲状腺癌与甲状腺腺瘤的鉴别诊断具有重要意义。
Objective To evaluate the value of two-dimensional and color doppler ultrasound (CDFI) in the diagnosis of thyroid cancer.Methods A total of 58 thyroid carcinomas and 47 thyroid adenomas confirmed by surgery and pathology were enrolled in our hospital from August 2015 to February 2018.The imaging features of two-dimensional and color doppler flow imaging were used to examine the different ultrasound findings of the patient, and the distribution of blood flow inside and around the lesion [peak velocity (Vmax) and resistance index (RI)] record analysis.Results The coincidence rate of ultrasound diagnosis in this study was 86.21%.The two-dimensional sonogram of thyroid cancer showed peripheral silent, hypoechoic and microcalcifications.,the detection rates were 87.93%,70.69% and 63.79%,respectively. Thyroid cancer the performance of the internal blood flow signal is mainly grade I and II, characterized by abundant blood flow, high flow rate and high resistance. There were significant differences in the two-dimensional sonograms and CDFI blood flow grading between the two groups (P<0.05).The Vmax and RI values of the thyroid cancer group were significantly higher than those of the thyroid gland, the difference was statistically significant(P<0.05).Conclusion Two-dimensional and color doppler ultrasonography is used to analyze the morphology, internal echo type and blood flow characteristics of thyroid cancer and thyroid adenoma. It is of great significance for the differential diagnosis of thyroid cancer and thyroid adenoma.
