慢性阻塞性肺病合并Ⅱ型呼吸衰竭家庭氧疗和家庭氧疗加无创辅助通气临床疗效分析

Clinical Efficacy of Family Oxygen Therapy and Home Oxygen Therapy Plus Non-invasive Assisted Ventilation in Patients with Chronic Obstructive Pulmonary Disease Complicated with Type Ⅱ Respiratory Failure

目的探讨慢性阻塞性肺病合并Ⅱ型呼吸衰竭家庭氧疗和家庭氧疗加无创辅助通气临床疗效。方法方便选取该院2013年5月—2018年5月收治的51例慢性阻塞性肺病合并Ⅱ型呼吸衰竭患者,随机分为对照组和观察组,对照组26例实施家庭氧疗,观察组25例行家庭氧疗加无创辅助通气,比较两组肺功能改善情况及治疗效果等。结果观察组平均每人急性发作次数为(0.88±0.11)次,对照组为(1.55±0.14)次,比较差异有统计学意义(t=5.192,P<0.05);观察组6-MWT距离(380.5±22.6)m显著长于对照组(320.6±21.5)m,比较差异有统计学意义(t=10.692,P<0.05);治疗后,观察组FEV1%水平为(66.0±4.85)%,FEV1水平为(1.98±0.04)L显著高于对照组,组间数据差异有统计学意义(t=5.261,4.274,P<0.05);治疗后,观察组PaCO_2水平为(55.7±5.07)mmHg显著低于对照组(65.6±5.84)mmHg,组间数据差异有统计学意义(t=5.624,P<0.05)。结论对慢性阻塞性肺病患者进行家庭氧疗加无创辅助通气,价值良好,值得推广。

Objective To investigate the clinical efficacy of family oxygen therapy and home oxygen therapy plus non-invasive assisted ventilation in patients with chronic obstructive pulmonary disease complicated with type II respiratory failure.Methods Convenient select a total of 51 patients with chronic obstructive pulmonary disease and type II respiratory failure admitted to our hospital from May 2013 to May 2018 were randomly divided into the control group and the observation group.The control group received 26 cases of home oxygen therapy and the observation group of 25 cases.Home oxygen therapy plus non-invasive assisted ventilation,comparison of lung function improvement and treatment effects.Results The average number of acute episodes per person in the observation group was(0.88±0.11)times,and that in the control group was(1.55±0.14)times.The difference was significant(t=5.192,P<0.05).The distance of the observation group was 6-MWT(380.5±22.6)m was significantly longer than the control group(320.6±21.5)m,and the difference was significant(t=10.692,P<0.05).After treatment,the FEV1%level of the observation group was(66.0±4.85)%,and the FEV1 level was(1.98±0.04)L was significantly higher than the control group,the difference between the groups was significant(t=5.261,4.274,P<0.05);after treatment,the observation group PaCO2 level was(55.7±5.07)mmHg was significantly lower than the control group of(65.6±5.84)mmHg,and the data between the groups were significantly different(t=5.624,P<0.05).Conclusion Home oxygen therapy plus non-invasive assisted ventilation for patients with chronic obstructive pulmonary disease is of good value and worth promoting.
