开颅夹闭与血管栓塞应用于脑动脉瘤中的疗效与安全性观察

Efficacy and Safety of Craniotomy and Vascular Embolization in Cerebral Aneurysms

目的探讨开颅夹闭与血管栓塞应用于脑动脉瘤中的疗效与安全性。方法回顾性分析2012年1月—2018年3月该院67例脑动脉瘤患者的临床资料,其中32例开颅夹闭术患者作为对照组,35例行血管栓塞术患者作为研究组。对比两组临床疗效、术后并发症与住院时间。结果研究组治疗效果良好的发生率91.43%,高于对照组62.50%(χ2=8.051,P<0.05);治疗效果差的发生率8.57%,低于对照组34.38%(χ2=6.734,P<0.05),研究组死亡率0.00%与对照组3.13%对比差异无统计学意义(χ2=2.039,P>0.05)。研究组术后并发症发生率为5.71%,低于对照组28.13%(χ2=6.118,P<0.05)。研究组住院时间为(8.85±1.52)d,低于对照组(12.50±1.40)d(t=5.685,P<0.05)。结论相较于开颅夹闭术,血管栓塞治疗脑动脉瘤的疗效与安全性更为理想,且加快了患者的术后康复速度。

Objective To investigate the efficacy and safety of craniotomy and vascular embolization in cerebral aneurysms.Methods The clinical data of 67 patients with cerebral aneurysms from January 2012 to March 2018 were retrospectively analyzed.Among them,32 patients with craniotomy were used as control group and 35 patients with vascular embolization as study group.The clinical efficacy,postoperative complications and length of hospital stay were compared between the two groups.Results The incidence of good treatment in the study group was 91.43%,which was higher than 62.50%in the control group(χ^2=8.051,P<0.05).The incidence of poor treatment was 8.57%,which was lower than that in the control group(34.38%)(χ^2=6.734,P<0.05),there was no significant difference between the study group mortality rate of 0.00%and the control group of 3.13%(χ^2=2.039,P >0.05).The postoperative complication rate was 5.71%in the study group,which was lower than 28.13%in the control group(χ^2=6.118,P<0.05).The hospital stay in the study group was(8.85±1.52)d,which was lower than that of the control group(12.50±1.40)d(t=5.685,P<0.05).Conclusion Compared with craniotomy,vascular embolization is more effective and safer in the treatment of cerebral aneurysms,and accelerates the postoperative recovery rate.
