糖尿病合并脑血管病70例临床分析

Clinical Analysis of 70 Cases of Diabetes Complicated with Cerebrovascular Disease

目的分析糖尿病合并脑血管病临床特征,总结疾病管理经验。方法方便选取2017年1—12月,神经内科住院糖尿病合并脑血管疾病70例入组,纳入观察组,一对一选择未合并2型糖尿病患者70例入选对照组,对比两组对象的合并症、病史、疾病类型、强化治疗措施开展情况、预后资料。结果观察组与对照组合并组、入院高血糖与高血压、梗死部位、梗死病史差异有统计学意义(P<0.05)。观察组神经营养、亚低温、高压氧治疗率、应激性溃疡、中重度残疾/植物生存/死亡、进展性脑卒中比重分别为97.14%、15.71%、25.71%、30.00%、11.43%、20.00%、15.71%高于对照组75.71%、1.43%、8.57%、5.71%、2.86%、7.14%、2.86%,差异有统计学意义(χ2=13.702、9.115、7.241、14.073、3.877、4.933、6.869,P=0.000、0.003、0.007、0.000、0.049、0.026、0.009<0.05)。结论糖尿病合并脑血管病病情更重,急性脑血管病比重高,对治疗的需求更强,且预后更差。

Objective To analyze the clinical features of diabetic cerebrovascular disease and summarize the experience of disease management.Methods From January to December 2017,70 patients with neurological hospitalized diabetes mellitus complicated with cerebrovascular disease were convenient selected and enrolled in the observation group.One-to-one selected patients with type 2 diabetes were enrolled in the control group,and the two groups were compared of disease,medical history,type of disease,intensive treatment measures,prognosis data.Results There were significant differences between the observation group and the control group,high blood glucose and hypertension,infarction site and infarction history(P<0.05).The neurotrophic,hypothermia,hyperbaric oxygen therapy,stress ulcer,moderate to severe disability/plant survival/death,and progressive stroke were 97.14%,15.71%,and 25.71%,30.00%,11.43%,20.00%,15.71%higher than the control group 75.71%,1.43%,8.57%,5.71%,2.86%,7.14%,2.86%,the difference was statistically significant(χ^2=13.702,9.115,7.241,14.073,3.877,4.933,6.869,P=0.000,0.003,0.007,0.000,0.049,0.026,0.009<0.05).Conclusion Diabetes complicated with cerebrovascular disease is more serious,and the proportion of acute cerebrovascular disease is high.The demand for treatment is stronger and the prognosis is worse.
