以问题为导向的针对性护理模式对胃癌根治术患者术后康复效果及生活质量的影响

Effect of problem-oriented targeted nursing model on postoperative rehabilitation and quality of life of patients with gastric cancer undergoing radical gastrectomy

目的探究以问题为导向的针对性护理模式对胃癌根治术患者术后康复效果及生活质量的影响。方法将本院行胃癌根治术的86例患者按随机数字表法分对照组(n=43,常规护理)与研究组(n=43,在对照组基础上行以问题为导向的针对性护理)。比较两组患者的护理效果。结果研究组术后肠鸣音恢复时间、肛门排气时间、腹胀改善时间、排便时间及住院时间均短于对照组(P<0.05)。干预后,两组患者自我概念、自护责任感、自护技能及健康知识水平评分均升高,且研究组高于对照组(P<0.05)。干预后,两组患者SAS及VAS评分均降低,且研究组低于对照组(P<0.05)。干预后,两组患者社会功能、躯体疼痛、生理职能、生理功能、情感职能、精神健康、精力、总体健康评分均升高,且研究组高于对照组(P<0.05)。结论胃癌根治术患者行以问题为导向的针对性护理可显著减轻疼痛与焦虑程度,促进术后康复,并能提高患者自我护理能力和生活质量,值得推广应用。

Objective To explore the effect of problem-oriented targeted nursing model on postoperative rehabilitation and quality of life of patients with gastric cancer undergoing radical gastrectomy.Methods Eighty-six gastric cancer patients undergoing radical gastrectomy in our hospital were divided into control group(n=43,routine nursing)and research group(n=43,problem-oriented targeted nursing on the basis of control group)according to random number table method.The nursing effects of the two groups were compared.Results The time of bowel sounds recovery,anal exhaust time,time of abdominal distension improvement,defecation time and hospitalization time in the research group were shorter than those in the control group(P<0.05).After intervention,the scores of self-concept,self-care responsibility,self-care skills and health knowledge in both groups increased,and those in the research group were higher than the control group(P<0.05).After intervention,the scores of SAS and VAS of both groups decreased,and those of the research group were lower than the control group(P<0.05).After intervention,the scores of social functioning,bodily pain,role-physical,physical functioning,role-emotional,mental health,vitality and general health of the two groups increased,and those of the research group were higher than the control group(P<0.05).Conclusion Problem-oriented targeted nursing for gastric cancer patients undergoing radical gastrectomy can significantly alleviate pain and anxiety,promote post-operative rehabilitation,and improve patients'self-care ability and quality of life,which is worthy of promotion and application.
