摘要
目的观察麻醉苏醒室(PACU)3 h内非湿化中流量(4 L/min)吸氧对减轻护士工作量、减少患者治疗费用以及患者术后舒适度的影响。方法观察508例全麻术后PACU中流量吸氧患者,其中非湿化250例与湿化组患者258例,比较入、出PACU时心率(HR)、呼吸频率(R)、血氧饱和度(SPO2)、收缩压(SBP)、舒张压(DBP)。患者Stward评分=6分后,评价其舒适度(如:鼻腔黏膜不适、上呼吸道干燥,氧异味等);比较两组吸氧方法治疗费用、护士安装与预处理氧气装置时间的差异。结果两组患者入、出PACU时心率(HR)、呼吸频率(R)、血氧饱和度(SPO2)、收缩压(SBP)、舒张压(DBP)比较差异无统计学意义,鼻腔黏膜不适评分、上呼吸道干燥情况比较差异无统计学意义,非湿化组患者治疗费用比湿化组减少34.1元,非湿化组护士安装及预处理时间少于湿化组(P<0.01)。结论 3 h内的4 L/min中流量非湿化与湿化吸氧对PACU患者的舒适度无显著差异,但可以减少患者治疗费用,减轻护士的工作量,减少环境污染,值得临床推广。
Objective To observe the influence on patient' s costs the time of nurses' installation and disposition and the patient' s comfort when patients in PACU by middle-flow(4 L/min)no humidified oxygen within three hours. Methods To observe HR, R, SPO2, SBP, DBP when arrived and were ready to leave PACU for 508 patients after general anesthesia operations, 250 patients who received middle-flow no humidified oxygen;258 patients who received middle-flow humidified oxygen;the discomfort condition in nasal mucosa and dry feeling in upper respiratory for patients;To compare the patient costs and the time of nurses' installation and disposition for two groups. Results To observe P, R, SPO2, SBP, DBP when arrived and were ready to leave PACU. It's no significant difference of the discomfort condition in nasal mucosa and dry feeling in upper respiratory between two groups. There is a reduction of 34.1 yuan for no humidified oxygen group. It' s significant difference between two groups (P<0.01) for installation and disposition. Conclusion There is no significant difference between two groups, who received middle-flow (4 L/ min) during three hours of patients in PACU no humidified oxygen within three hours in PACU or humidified oxygen. It can reduce patients' costs, the nurses' workload and environmental pollution by no humidified oxygen, it's worthy of clinical promotion.
