摘要
目的研究两种血管通路在尿毒症患者血液透析中的应用效果比较。方法回顾性分析2014年1月至2016年12月在本院治疗的50例尿毒症患者,依据不同血管通路方式的分为对照组和观察组,各25例,其中对照组行长期深静脉置管模式,观察组行动静脉内瘘模式,比较两组患者的Hb、ALB、IL-6、CRP水平,治疗满意度,并发症发生情况等。结果治疗后,观察组Hb、ALB水平升高,IL-6、CRP水平降低,观察组Hb、ALB水平较对照组高,IL-6、CRP水平较对照组低,两组差异有统计学意义(P<0.05);观察组治疗满意度92%,较对照组64%高,且观察组并发症仅出现1例出血,发生率4%,对照组有9例并发症,发生率36%,两组差异有统计学意义(P<0.05)。结论动静脉内瘘可提高尿毒症血液透析患者的治疗满意度,避免并发症,对于患者康复具有重要意义。
Objective To study the application effect of two vascular pathways in hemodialysis of uremic patients. Methods We retrospectively analyzed 50 patients with uremia who were treated in our hospital from January 2014 to December 2016. According to different vascular access methods, they were divided into control group and observation group with 25 cases in each group. The control group was treated with long-term deep venous placement. In the tube mode, the observation group took a venous fistula pattern and compared the levels of Hb, ALB, IL-6, CRP, satisfaction of treatment, and complications between the two groups. Results After treatment, the levels of Hb and ALB in the observation group increased, the levels of IL-6 and CRP decreased, the levels of Hb and ALB in the observation group were higher than those in the control group, and the levels of IL-6 and CRP were lower in the observation group than in the control group, the difference was statistically significant between the two groups (P<0.05);92% of the patients in the observation group were satisfied with the treatment, which was 64% higher than that of the control group. In the observation group, there was only one case of complications and the incidence was 4%. In the control group, there were 9 cases of complications and the incidence was 36%. There was a statistically significant difference between the two groups (P<0.05). Conclusion Arteriovenous fistula can improve the treatment satisfaction of uremic hemodialysis patients, avoid complications, and have important significance for the rehabilitation of patients.
