同期手术治疗混合痔合并低位肛瘘的临床疗效及安全性

Clinical Efficacy and Safety of Simultaneous Operation for Mixed Hemorrhoid Combined with Low Anal fistula

目的探讨同期手术治疗混合痔合并低位肛瘘的临床疗效及安全性。方法选取于2015年5月至2017年5月收治我院的80例混合痔合并低位肛瘘患者,随机分成对照组和观察组,每组40例。观察组患者予以同期手术治疗混合痔以及低位肛瘘,对照组患者在手术治疗低位肛瘘痊愈后再继续进行混合痔手术治疗。治疗3个月后,对两组患者手术情况、临床疗效、手术并发症以及肛门功能进行比较。结果观察组患者的手术时间、术中出血量、切口愈合时间、肛门功能评分均较对照组患者显著改善,差异有统计学意义(t=6.082,17.715,6.612,10.000;P均<0.01);观察组患者的总有效率显著高于对照组患者,差异有统计学意义(c2=7.257;P=0.026);观察组患者在术后疼痛、排尿障碍、切口感染等并发症的发生率上均显著低于对照组患者且差异有统计学意义(c2=4.831,3.914,6.275,5.165;P=0.028,0.048,0.012,0.023)。结论同期对患者的混合痔合并肛瘘进行治疗,能够有效地提高临床疗效,减少并发症并且减轻患者痛苦,是一种安全有效的治疗方法。

Objective To evaluate the efficacy and safety of simultaneous operation for mixed hemorrhoid and low anal fistula. Methods From May 2015 to May 2017, 80 patients with mixed hemorrhoid and low anal fistula were admitted to our hospital. They were randomly divided into the control group and the observation group, each group 40 people. The mixed hemorrhoid in the observation group was operated on at the same time as the low anal fistula, while the control group was treated by stages. After 3 months of treatment, the operation, clinical efficacy, surgical complications and anal function of the two groups were compared. Results The operation time, blood loss, wound healing time and anal function score of the observation group were significantly improved compared with the patients in the observation group, the difference was statistically significant (t=6.082, 17.715,6.612,10.000;P<0.01);The observation group’s total effective rate was significantly higher than that of the control group (c2=7.257, P=0.026);The patients in the observation group had pain after operation. The incidence of complications such as voiding dysfunction, incision infection and so on were lower than those in the control group (c2=4.831,3.914,6.275,5.165;P=0.028,0.048,0.012,0.023). Conclusion The treatment of mixed hemorrhoid and anal fistula in the same period can not only effectively improve the clinical efficacy, but also reduce the incidence of complications and relieve the pain of patients. It is a safe and effective treatment.
