小细胞肺癌患者血清及胸水中ProGRP表达水平及临床意义

摘要 目的探讨小细胞肺癌患者血清及胸水中胃泌素释放肽前体(ProGRP)表达水平及临床意义。方法选择本院2014年1月至2016年12月收治的200例小细胞肺癌患者为研究对象(观察组),另选择同期本院收治肺部炎症患者200例作为对照组,分别采用罗氏电化学发光自动分析仪检测血清和胸水ProGRP水平。结果观察组血清及胸水ProGRP水平均高于对照组(P<0.05),且观察组胸水ProGRP水平高于血清ProGRP水平(P<0.05);观察组广泛期血清及胸水ProGRP水平均明显高于局限期(P<0.05),且观察组各期胸水ProGRP水平高于血清ProGRP水平(P<0.05);观察组血清ProGRP检测灵敏度低于胸水ProGRP检测(P<0.05);治疗后缓解期、好转期胸水ProGRP水平降低(P<0.05),进展期胸水ProGRP水平升高(P<0.05),胸水ProGRP水平:缓解期<好转期<进展期(P<0.05)。结论小细胞肺癌患者胸水中ProGRP表达水平高于血清中ProGRP表达水平,对小细胞肺癌诊断、分期判断及预后均有较高临床价值。 Objective To investigate the expression level and clinical significance of pro-gastrin-releasing peptide (ProGRP) in serum and pleural effusion of patients with small cell lung cancer. Methods A total of 200 patients with small cell lung cancer admitted to the hospital from January 2014 to December 2016 were enrolled as study objects (observation group). Another 200 patients with pulmonary inflammation in the hospital during same period were selected as the control group. Roche automatic electrochemiluminescence analyzer was performed to detect ProGRP levels in serum and pleural effusion. Results The ProGRP levels in serum and pleural effusion of the observation group were higher than those of the control group (P<0.05). The ProGRP level in pleural effusion of the observation group was higher than that in the serum (P<0.05). The ProGRP levels in serum and pleural effusion during extensive stage in the observation group were significantly higher than those during limited stage. The ProGRP levels in pleural effusion during all stages of the observation group were higher than those in the serum (P<0.05). The sensitivity for detecting serum ProGRP in the observation group was lower than that for detecting pleural effusion ProGRP (P<0.05). The levels of ProGRP in pleural effusion during remission period and improvement period were decreased after treatment (P<0.05), while the levels of ProGRP in pleural effusion during advanced stage increased (P<0.05). The level of ProGRP in pleural effusion during remission period was the least, followed by improvement period and advanced period (P<0.05). Conclusion The expression level of ProGRP in pleural effusion of patients with small cell lung cancer is higher than that in serum, which has high clinical value for the diagnosis, staging judging and prognosis of small cell lung cancer.
