比较全身麻醉与椎管内麻醉对高龄股骨粗隆间骨折患者术后恢复效果的影响

Effect of general anesthesia and intraspinal anesthesia on postoperative recovery of elderly patients with intertrochanteric fracture of femur

目的比较分析全身麻醉与椎管内麻醉对高龄股骨粗隆间骨折患者术后恢复效果的影响。方法选取2015年1月至2018年1月期间来本院接受手术治疗的80例高龄股骨粗隆间骨折患者选为研究对象,依据随机抽签法分为两组,即对照组与实验组,各组患者40例。对照组患者予以全身麻醉,实验组患者予以椎管内麻醉,统计对比两组患者手术时间、术中出血量、术后下床活动时间、骨折愈合时间、住院时间及并发症发生率。结果两组患者手术时间、术中出血量比较差异无统计学意义。实验组患者术后下床活动时间、骨折愈合时间、住院时间显著短于对照组患者,差异有统计学意义(P<0.05)。实验组患者并发症发生率显著低于对照组(7.50%vs 25.00%),差异具有统计学意义(P<0.05)。结论高龄股骨粗隆间骨折患者应用椎管内麻醉的临床效果更加确切,能够有效加快患者术后恢复,减少并发症,临床应用价值非常高。

objective To compare the effects of general anesthesia and intraspinal anesthesia on postoperative recovery of elderly patients with intertrochanteric fracture of femur. Methods From January 2015 to January 2018, 80 elderly patients with intertrochanteric fracture of femur were selected as study objects. According to the method of random drawing, they were divided into two groups: control group and experimental group, 40 cases in each group. The patients in the control group were given general anesthesia, and the patients in the experimental group were given intraspinal anesthesia. The operation time, the amount of blood lost during the operation, the time of getting out of bed after operation, the time of fracture healing, the time of hospitalization and the incidence of complications were compared statistically between the two groups. Results There was no significant difference in operative time and intraoperative bleeding between the two groups. The time of getting out of bed, the time of fracture healing and the time of hospitalization were significantly shorter in the trial group than in the control group (P<0.05). The incidence of complications in the trial group was significantly lower than that in the control group (7.50% vs 25.00%, P<0.05). Conclusion The clinical effect of intraspinal anesthesia in elderly patients with intertrochanteric fracture is more accurate, which can accelerate the postoperative recovery and reduce the complications. The clinical application value is very high.
