纳洛酮联合无创正压通气治疗慢性阻塞性肺疾病合并呼吸衰竭的临床效果 被引量:5

Clinical effect of naloxone combined with noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease with respiratory failure

摘要 目的总结纳洛酮联合无创正压通气治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床疗效。方法选择2015年2月至2017年12月本院收治的90例COPD合并呼吸衰竭患者作为研究对象,遵循数字表随机原则分为两组,各45例,对照组接受无创正压通气治疗,每4小时1次;观察组在其基础上展开纳洛酮静脉注射治疗;两组均为期7 d的疗程,比较两组疗效与血气分析指标水平变化。结果分析组与对照组治疗总有效率差异存在统计学意义(95.56%vs 82.22%)(P<0.05);两组治疗前血氧分压、血氧饱和度、pH值、二氧化碳分压水平差异无统计学意义,经分组治疗后获得改善,且分析组改善效果明显优于对照组,差异具有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(6.67%vs 4.44%)。结论纳洛酮联合无创正压通气治疗COPD联合呼吸衰竭具有较好效果,有助于改善提升临床疗效,改善患者血气分析水平,且安全性较高,值得临床推广应用。 Objective To summarize the clinical efficacy of naloxone combined with noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease (COPD) with respiratory failure. Methods Ninety patients with COPD and respiratory failure admitted to our hospital from February 2015 to December 2017 were enrolled in the study. They were divided into two groups according to the randomized principle of digital table. The control group received noninvasive positive pressure ventilation treatment, 1 time/4h;the observation group was based on the intravenous injection of naloxone;both groups were on the 7th course of treatment, comparing the two groups of efficacy and blood gas analysis indicators. Results There was statistically significant difference in the total effective rate between the analysis group and the control group (95.56% vs 82.22%)(P<0.05). The blood oxygen partial pressure, blood oxygen saturation and pH value before treatment. There was no significant difference in the partial pressure of carbon dioxide, which was improved after group treatment, and the improvement effect of the analysis group was significantly better than that of the control group. The difference was statistically significant (P<0.05). There was no significant difference in the incidence rate (6.67% vs 4.44%). Conclusion Naloxone combined with non-invasive positive pressure ventilation has a good effect in the treatment of COPD combined with respiratory failure. It can improve the clinical efficacy and improve the blood gas analysis level of patients. It is safe and worthy of clinical application.
