慢性萎缩性胃炎胃镜下不同病理改变及幽门螺杆菌感染的相关研究

Study on different pathological changes and helicobacter pylori infection under gastroscopy in chronic atrophic gastritis

目的探究慢性萎缩性胃炎(Chronic Atrophic Gastritis,CAG)在胃镜下的不同病理改变和幽门螺杆菌(Helicobacter pylo-ri,Hp)感染之间的关系。方法选取2017年1月至2018年1月收治的126例CAG患者临床资料进行分析,经胃黏膜活组织病理检查确诊,均予以胃镜检查,CAG内镜下特征分型检出结果,分成A、B两类,分析胃镜下不同病理改变情况,并观察其与Hp感染的关系。结果 126例CAG患者内镜下检出A类74例,检出率为58.73%;B类检出52例,检出率为41.26%;B类患者诊断符合率92.31%高于A类患者78.37%,比较差异具有统计学意义(P<0.05),且B类患者病理结果显示不典型增生检出率23.07%、肠上皮化生检出率25.00%高于A类患者9.45%、10.81%,比较差异具有统计学意义(P<0.05);A类患者Hp阳性感染率58.10%低于B类患者76.92%,比较差异具有统计学意义(P<0.05)。结论 CAG患者在胃镜检查下显示A类例数较B类多,与病理检查结果相比B类诊断符合率更高,且Hp阳性感染率更高,临床应加强对其诊断及治疗。

Objective To investigate the relationship between Chronic Atrophic Gastritis (CAG) and Helicobacter pylori (Hp) infection. Methods 126 patients with CAG admitted and treated from January 2017 to January 2018 were analyzed. All of them were confirmed by gastric mucosal biopsy and gastroscopy. The results of CAG endoscopic feature typing were divided into two categories: A and B. Results The endoscopic detection of class A in 74 patients with 126 CAG patients was 58.73%. 52 cases were detected in category B, with a detection rate of 41.26%. The diagnostic coincidence rate of class B patients was 92.31% higher than that of class A patients 78.37%, with statistically significant difference (P<0.05). The pathological results of class B patients showed that the detection rate of atypical hyperplasia was 23.07%, and the detection rate of intestinal metaplasia was 25.00% higher than that of class A patients 9.45% and 10.81%, with statistically significant difference (P<0.05). The Hp positive infection rate of class A patients was 58.10% lower than that of class B patients was 76.92%, and the difference was statistically significant (P<0.05). Conclusion The number of patients with CAG under gastroscopy is more than that of patients with CAG under gastroscopy. Compared with the pathological examination results, the diagnosis coincidence rate of patients with CAG is higher than that of patients with CAG under gastroscopy. In addition, the Hp positive infection rate is higher.
