囊性小肾癌的MRI表现与Bosniak分级的相关性分析

Correlation Analysis between MRI Findings and Bosniak Classification of Cystic SmallRenal Carcinoma

目的探讨囊性小肾癌的MRI表现与Bosniak分级的相关性。方法以78例囊性小肾癌患者作为研究对象,记录所有患者的MRI表现与Bosniak分级情况;同时对患者进行手术病理Bosniak分级与相关性分析。结果手术病理确诊为BosniakⅠ级25例,Ⅱ级20例,Ⅲ级25例,Ⅲ级8例。MRI判断为囊壁/分隔增厚21例,钙化32例,实性成分18例; BosniakⅠ级24例,Ⅱ级18例,Ⅲ级28例,Ⅲ级8例,MRI诊断囊性小肾癌Bosniak分级的准确性为93. 6%。MRI T2WI检查表现为60例病灶边界清楚,T2WI显示60例病灶囊内呈高信号,DWI显示58例病灶囊内均呈低信号,增强扫描显示40例病灶囊壁无强化。直线相关性分析显示:MRI囊壁/分隔增厚、钙化、实性成分都与手术病理Bosniak分级呈明显正相关性(P <0. 05)。结论囊性小肾癌的MRI表现与Bosniak分级有明显的相关性,对囊性小肾癌的病情判断与诊断具有重要意义,也可为临床治疗提供可靠的参考依据。

Objective To investigate the correlation between MRI findings and Bosniak classification of cystic small renal carcinoma.Methods Used a retrospective study,78 patients of cystic small renal carcinoma in our hospital for treatment were selected as the research object,the MRI and Bosniak classification were recorded in all patients;and the pathological Bosniak classification were given analyzed and correlation at the same time.Results In the 78 patients,there were 25 patients were diagnosed as Bosniak gradeⅠ,20 patients were gradeⅡ,25 patients were gradeⅢand 8 patients were gradeⅣrespectively.MRI were diagnosed 21 patients of cystic wall/compartment thickening,32 patients were calcification,18 patients were solid component,there were 24 patients were Bosniak grade I,18 patients were gradeⅡ,28 patients were gradeⅢand 8 patients were gradeⅣrespectively,so the MRI in the diagnosis of Bosniak classification accuracy was 93.6%.MRI T2WI examination showed 60 patients with clear boundary,and T2WI showed 60 patients were high signal.DWI showed low signal of 58 patients,and the enhance scan showed no enhancement of cystic wall in 40 patients.The linear correlation analysis showed that the MRI cyst wall/partition thickening,calcification and solid components were positively correlated with pathological Bosniak classification(P<0.05).Conclusion The MRI findings of cystic small renal cell carcinoma are closely related to the Bosniak classification,which are important significance for the diagnosis and diagnosis of cystic small renal carcinoma,and it can provide a reliable reference for clinical treatment.
