厄贝沙坦联合小剂量利尿剂对老年单纯收缩期高血压患者血清炎症因子及血管内皮功能的影响

Effect of irbesartan tablets combined with a small dose of diuretics on serum inflammatory factors and vascular endothelial function in elderly patients with isolated systolic hypertension

目的研究厄贝沙坦联合小剂量利尿剂对老年单纯收缩期高血压患者对其血清炎症因子及血管内皮功能影响。方法将本院收治的老年单纯收缩期高血压患者204例,按照随机数表法平均分为2组。其中对照组102例,给予小剂量氢氯噻嗪片;观察组在对照组基础上联合厄贝沙坦片。对比治疗前和治疗4周后2组患者的收缩压和舒张压、血清炎症因子、血管内皮功能指标、不良反应及临床疗效。结果治疗后,2组患者内皮素-1(ET-1)降低而一氧化氮(NO)增加(P <0. 05),且观察组NO高于对照组而ET-1低于对照组(P <0. 05)。治疗后,2组患者超敏C反应蛋白(hs-CRP)及可溶性细胞间黏附分子-1(s ICAM-1)均降低(P <0. 05),且观察组低于对照组(P <0. 05)。2组患者不良反应发生率相比,差异无统计学意义(P> 0. 05)。但观察组治疗总有效率为94. 1%,高于对照组的74. 5%(P <0. 05)。结论厄贝沙坦联合小剂量利尿剂治疗老年单纯收缩期高血压,可有效改善其舒张压和收缩压,改善血清炎症因子及血管内皮功能,具有疗效好、不良反应低等优点。

Objective To study the effect of irbesartan combined with small dose of diuretics on serum inflammatory factors and vascular endothelial function in elderly patients with isolated systolic hypertension.Methods A total of 204 elderly isolated systolic hypertension patients were randomly divided into 2 groups.The control group with 102 cases was given a small dose of Hydrochlorothiazide Tablets,while the observation group additionally given Irbesartan Tablets on the basis of the control group.The systolic blood pressure and diastolic blood pressure,serum inflammatory factors,vascular endothelial function indexes,adverse reactions and clinical efficacy were compared before treatment and after 4-week treatment between the two groups.Results After treatment,endothelin-1(ET-1)decreased and nitric oxide(NO)increased in the two groups(P<0.05),and NO in the observation group was higher,while ET-1 was lower than that in the control group(P<0.05).After treatment,high-sensitivity C-reactive protein(hs-CRP)and soluble inter-cellular adhesion molecule-1(sICAM-1)of the two groups were decreased(P<0.05),and the observation group was lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).But the total effective rate of the observation group was 94.1%,which was higher than 74.5%of the control group(P<0.05).Conclusion Irbesartan combined with a small dose of diuretics in the treatment of the elderly with isolated systolic hypertension can effectively improve diastolic blood pressure and systolic blood pressure,improve vascular endothelial function and inflammatory factor,which has good curative effect,and lower adverse reactions.
