老年髋部骨折患者骨密度与骨碱性磷酸酶、Ⅰ型胶原羧基端前肽、Ⅰ型胶原羧基端交联肽的相关性 被引量:17

摘要 目的探讨老年髋部骨折患者骨密度与骨碱性磷酸酶(BALP)、Ⅰ型胶原羧基端前肽(CICP)、Ⅰ型胶原羧基端交联肽(CTX-Ⅰ)表达的相关性。方法选取110例老年髋部骨折患者设为病例组,另选取未骨折的110例老年患者设为对照组,比较2组患者骨密度及骨代谢指标的差异,分析骨密度与骨代谢指标BALP、CICP、CTX-Ⅰ的相关性。结果 2组患者腰椎均值及股骨颈的骨密度比较,差异无统计学意义(P>0.05);对照组粗隆、粗隆间及总髋部的骨密度均显著高于病例组(P<0.05);病例组患者的BALP、CICP、CTX-Ⅰ均显著高于对照组(P<0.05);腰椎均值和股骨颈的骨密度与BALP、CICP、CTX-Ⅰ不具有相关性(P>0.05);粗隆、粗隆间、总髋部的骨密度均与BALP、CICP、CTX-Ⅰ具有负相关性(P<0.01)。结论老年髋部骨折患者不同部位的骨密度与骨代谢指标BALP、CICP、CTX-Ⅰ有相关性,应在患者早期骨代谢水平变化时给予一定的预防治疗,以降低老年骨质疏松患者发生髋部骨折的可能性。 Objective To study the correlation between bone mineral density(BMP),bone alkaline phosphatase(BALP),typeⅠcollagen carboxy-terminal propeptide(CICP)and typeⅠcollagen carboxy-terminal cross-linking peptide(CTX-Ⅰ)in elderly patients with hip fracture.Methods A total of 110 elderly patients with hip fractures were selected as case group,and another 110 elder patients without fracture was selected as control group.The differences of bone mineral density and bone metabolism index between the two groups were compared.The correlation between bone mineral density and bone metabolic markers such as BALP,CICP and CTX-Ⅰwas analyzed.Results There was no significant difference in the mean value of lumbar vertebrae and bone mineral density in the femoral neck between the two groups(P>0.05).The bone density of trochanter,intertrochanter and total hip was higher in the control group than the case group(P<0.05).BALP,CICP,and CTX-Ⅰin the case group were higher than control group(P<0.05).There was no significant correlation between the density of mean lumbar spine and the femoral density in BALP,CICP,and CTX-Ⅰ.The bone density of trochanter,intertrochanter and total hip was negatively correlated with BALP,CICP,and CTX-Ⅰ(P<0.05).Conclusion There is a correlation between bone mineral density and bone metabolic markers including BALP,CICP and CTX-Ⅰin elderly patients with hip fracture.So,preventive treatment should be given in the early stage of hip fracture patients to reduce the incidence of senile osteoporosis.
