肺保护性通气策略对单肺通气大鼠肺组织中水通道蛋白表达的影响 被引量:6

Influence of pulmonary protective ventilation strategy on aquaporin expression in lung tissues of rats with one-lung ventilation

摘要 目的探讨肺保护性通气策略对水通道蛋白(AQP)表达的影响。方法将15只成年雄性SD大鼠随机分为对照组(C组)、肺保护通气组(P组)、大潮气量组(H组),每组5只。C组仅切开气管导管并保留自主呼吸,P组与H组行气管插管后连接小动物呼吸机行左肺机械通气。C组在气管切开即刻处死大鼠,P组与H组在机械通气4 h后处死大鼠。采用免疫组化法测定左肺组织AQP蛋白表达。采用RT-PCR法测定AQP mRNA的表达。将剩余左肺组织用于计算肺组织湿/干重比值。结果免疫组化结果显示,C组、P组与H组AQP-1蛋白表达均未见明显差异,H组AQP-5蛋白表达较其他2组显著减少。P组和H组AQP-1 mRNA、AQP-5 mRNA表达均小于C组,且P组高于H组(P<0.05)。P组和H组中肺组织湿/干比值均显著大于C组(P<0.01),P组和H组肺组织湿/干比值有显著差异(P<0.01)。结论肺保护性通气策略能减少单肺通气大鼠肺组织中AQP-1和AQP-5蛋白的表达,但与传统的大潮气量通气模式相比,其影响相对较小。 Objective To explore the influence of pulmonary protective ventilation strategy on the expression of aquaporins(AQP).Methods A total of 15 SD male rats were randomly divided into control group(group C),pulmonary protective ventilation group(group P)and high-volume ventilation group(group H),with 5 rats in each group.Rats in group C only incised tracheal tube and retained spontaneous breathing,while rats in group P and group H received tracheal intubation and connected ventilator for smallanimals for left lung ventilation.Rats in group C were sacrificed immediately after tracheotomy,and rats in group P and group H were sacrificed 4 hours after mechanical ventilation.The expression of AQP protein in left lung tissue was determined by immunohistochemistry.The expression of AQP mRNA was determined by RT-PCR.The remaining left lung tissue was used to calculate the ratio of wet weight to dry weight.Results Immunohistochemical results showed that there was no significant difference in the expression of AQP-1 among group C,group P and group H.The expression of AQP-5 in group H was significantly lower than that in other two groups.The expressions of AQP-1 mRNA and AQP-5 mRNA in group P and H were significantly lower than those in group C,and the expressions of AQP-1 and AQP-5 in group P were significantly higher than those in group H(P<0.05).The ratios of wet weight to dry weight in lung tissue in group P and H were significantly higher than those in group C(P<0.01),and ratio of wet weight to dry weight in lung tissue also showed significant difference between group P and group H(P<0.01).Conclusion Pulmonary protective ventilation strategy can reduce the expression of AQP-1 and AQP-5 protein in lung tissue of rats with one-lung ventilation,but its effect is relatively small when compared with the traditional high tidal volume ventilation mode.
