扩散张量成像在精神分裂症患者脑白质纤维FA值检测中的临床研究

Clinical study of diffusion tensor imaging in white matter of patients with schizophrenia

目的:运用扩散张量成像分析精神分裂症患者脑白质纤维FA值量的改变,探讨医学影像学定性诊断依据。方法:收治精神分裂症患者和健康者各50例,均做MRI常规检查并行扩散张量成像,比较颅内各解剖结构FA值变化特点。结果:患者组与正常组各部位FA值分布差异有统计学意义(P<0.05)。右侧颞叶中心、双侧内囊前肢脑白质纤维FA值较正常组高。双顶叶中心、双侧颞上回、双侧扣带前部、胼胝体膝部、双侧小脑半球的脑白质纤维FA值较正常组少。结论:扩散张量成像在精神分裂症患者脑白质纤维中的FA值量的改变存在不对称性,只依靠大脑某部位局部FA值量的改变,还不能为精神分裂症提供参考依据,只能作为精神分裂症患者某临床症状的参考。

Objective:Diffusion tensor imaging was used to analyze the changes of FA value of white matter fibers in patients with schizophrenia and to explore the basis for qualitative diagnosis of medical imaging.Methods:50 patients with schizophrenia and 50 healthy persons were examined by routine MRI and diffusion tensor imaging, and the changes of FA values of intracranial anatomical structures were compared.Results:There was significant difference in FA value distribution between the patients group and the normal group(P<0.05).The FA values of white matter fibers in the right central temporal lobe and bilateral anterior limbs of internal capsule were higher than those in the normal group.FA values of white matter fibers in bilateral parietal lobe center, bilateral superior temporal gyrus,bilateral anterior cingulate,genu of corpus callosum and bilateral cerebellar hemisphere were less than those in the normal group.Conclusion:Diffusion tensor imaging has asymmetry in the change of FA value in white matter fiber of patients with schizophrenia.It only depends on the change of local FA value in a certain part of the brain,and it can not provide reference for schizophrenia,which can be selected as a reference for a clinical symptom of schizophrenia patients.
