手部骨纤维异样增殖症的诊治

Fibrous dysplasia of hand:a report of 6 cases

目的探讨手部骨纤维异样增殖症的病因、病变特点及治疗方法。方法回顾分析1980年3月至2016年1月天津市天津医院采用病灶刮除并植骨治疗的6例手部骨纤维异样增殖症患者的临床资料。结果其中男1例,女5例;年龄6～27岁,平均15.7岁。病变部位包括掌骨1例,近节指骨2例,中节指骨1例,小指系列掌骨和近、中、末节指骨1例以及环、小指系列掌、指骨1例。术后随访30～56个月,平均41个月。1例术后22个月复发,再次行刮除病灶后植骨治疗,再次手术后随访33个月未见复发。末次随访手指总活动度(TAM)系统评定结果为优5例、良1例。结论手部骨纤维异样增殖症病因不明确。病灶刮除并植骨是治疗手部骨纤维异样增殖症的有效方法。

Objective To investigate the etiology,pathological features and treatment of fibrous dysplasia of bone of hand.Methods Clinical data of 6 patients with fiber dysplasia of bone of hand treated by focal bone clearance and bone grafting in Tianjin Hospital were retrospectively analyzed from March 1980 to January 2016.Results All cases included one male and 5 females with an average age of 15.7(6-27)years.The lesion sites included metacarpal in one case,proximal phalanx in 2 cases,middle phalanx in one case,little finger series metacarpal and proximal,middle and terminal phalanx in one case and ring,little finger series metacarpal and phalanx in one case.Postoperative follow-up ranged from 30 to 56 months,with an average of 41 months.One patient relapsed 22 months after surgery,and were treated with bone grafting after resection of the lesion.Total active motion was excellent in 5 cases and good in one case.Conclusion The etiology of fibrous dysplasia of bone of hand is not clear.Focal bone clearance and bone grafting is effective for fibrous dysplasia of bone of hand.
