维持性血液透析患者症状负担及生活质量之间的相关性分析

Symptom burden and life quality of maintenance hemodialysis patients: a correlative study

目的调查维持性血液透析(maintenance hemodialysis,MHD)患者症状负担现状及生活质量,分析两者间的相关性,为临床症状管理策略的制定提供依据。方法采用透析患者症状负担调查量表(DFSSBI量表),评估血液净化中心108例MHD患者的症状负担状况,应用SF-36量表评估患者生活质量。采用Spearman分析症状负担与生活质量之间的相关性。结果所有患者均出现症状负担,其中频繁程度最高的为尿毒症一般症状群,严重程度最重的为消化道症状群,困扰程度最重的为情感症状群。与健康人群相比,MHD患者生活质量的八个维度均降低,差异有统计学意义(P<0.05)。Spearman相关性分析结果显示生活质量与症状负担成负相关性(P<0.05)。结论 MHD患者同时出现多个症状,生活质量较差,减轻症状负担可提高患者生活质量。

Objective To find out the correlation of symptom burden to the quality of life of patients with maintenance hemodialysis (MHD) with a view to obtaining scientific knowledgefor the development of effective clinical symptom management strategies. Methods Firstly, dialysisfrequency severity and symptom burden index (DFSSBI) was used to assess the symptom burden status of the 108 subjects. The SF-36 scale was used to assess the quality of life of the MHD patients. Spearman was used to analyze the correlation between symptom burden and quality of life. Results Symptom burden was observed in all patients. The most frequent one was the general symptom group of uremia while the most serious one was the gastrointestinal symptom group. The most severely affected one was the emotional symptom group. Compared with those of the healthy people, the level of life quality as represented by the eight dimensions of the MHD patients was lower that that of the healthy people. The difference was significantly different ( P <0.05). Spearman analysis indicated that the quality of life was negatively correlated with the burden of symptoms ( P <0.05). Conclusions MHD patients have multiple symptoms at the same time, which give rise to poor quality of life. Reducing the burden of symptoms can effectively improve the quality of life of MHD patients.
