我院糖尿病并发感染患者抗菌药物应用分析

Application analysis of antimicrobial drugs in patients with diabetes mellitus complicated with infection in our hospital

目的:分析我院糖尿病患者伴发感染时抗菌药物使用情况,旨为糖尿病患者抗感染治疗提供依据。方法:采用回顾性调查方法,统计2017年6月–2018年6月糖尿病伴发感染患者抗菌药物使用品种、频次、调整频次、消耗金额、使用天数,对数据进行统计分析。结果:统计期间,共收集病例245例,有明确抗菌药物使用指征者241例(98.36%),抗菌药物使用基本符合要求;抗菌药物单药治疗143例,使用两种抗菌药物的有65例,以单一治疗为主,喹诺酮类使用频次最高(35.49%);抗菌药物二联用药中,使用频次最高的组合是β-内酰胺酶抑制剂复合制剂+喹诺酮类,使用频次最高为哌拉西林他唑巴坦;部分病例在抗菌药物治疗过程中调整治疗方案1～2次,平均住院天数8.91 d;抗菌药物平均使用金额1 462.44元。结论:我院对于糖尿病合并感染患者抗菌药物治疗以喹诺酮类与β-内酰胺酶抑制剂复合制剂为主,在治疗过程中也存在一定的不合理使用情况,需加强教育与监管。

Objective: To analyze the use of antimicrobials in diabetic patients with infection in our hospital, and provide evidences for anti-infective treatment of diabetic patients. Methods: A retrospective survey was conducted to analyze the variety, frequency, frequency of adjustment, consumption amount, and the number of days of using antimicrobials in patients with diabetes mellitus from June 2017 to June 2018. Results: A total of 245 cases were collected, 241 cases (98.36%) had the definite indications for using antimicrobials. About 143 cases were treated with single antimicrobial, and 65 cases were treated with two kinds of antimicrobials;quinolones were used most frequently, accounting for 35.49%;the combination of compound preparation of betalactamase inhibitors and quinolones was the most frequently applied combination of the two antimicrobials, the most frequently used was piperacillin-tazobactam;some patients adjusted the treatment regimen 1 – 2 times during the course of antimicrobial treatment, with an average 8.91 hospitalization days;the average amount of antimicrobial use was 1 462.44 yuan. Conclusion: Quinolones and compound preparation of beta-lactamase inhibitors were the main antimicrobial agents for diabetic patients with infection, which conform to the guidelines recommended principles. But there were some irrational uses of antimicrobial drugs in the course of treatment, so education and management should be strengthened.
