Changes and Significance of Serum CA199, LAP and GGT in Patients With Early Pancreatic Cancer

摘要 目的探讨早期胰腺癌患者血清亮氨酸氨基肽酶(LAP)、r-谷氨酰转移酶(GGT)和CA199的变化及临床意义。方法对43例早期胰腺癌组、38例胰腺炎组和40例健康对照组血清分别采用速率法、酶法、发光法检测LAP、GGT和CA199含量。结果早期胰腺癌组LAP、GGT、CA199含量分别为(54.07±17.49)U/L、(138.25±45.69)U/L、(447.53±133.84)U/mL,高于胰腺炎组和对照组,差异均有统计学意义(P <0.05),但对于LAP胰腺炎组和对照组差异无统计学意义(P> 0.05),而对于GGT和CA199含量,胰腺炎组明显高于对照组(P <0.05);对于早期胰腺癌患者单项的敏感性最高的为GGT,敏感性为97.73%,特异性最高的为LAP,特异性为93.02%,联合检测的敏感性、特异性及约登指数均高于单项检测。结论 LAP、GGT、CA199联合检测对于早期胰腺癌的诊断具有重要价值。 Objective To investigate the changes and clinical significance of serum leucine aminopeptidase(LAP),r-glutamyltransferase(GGT)and CA199 in patients with early pancreatic cancer.Methods The serum levels of LAP,GGT and CA199 were detected by rate,enzymatic and luminescence methods in 43 patients with early pancreatic cancer,38 patients with pancreatitis and 40 healthy controls.Results The levels of LAP,GGT and CA199 were(54.07±17.49)U/L,(138.25±45.69)U/L and(447.53±133.84)U/mL respectively in early pancreatic cancer group,which were higher than those in the pancreas group and the control group,the difference was statistically significant(P<0.05),but there was no significant difference between LAP in pancreatitis group and control group(P>0.05),but for GGT and CA199 content,pancreatitis group was also significantly higher than control group(P<0.05);for early pancreatic cancer patients,the highest sensitivity of single item was 97.73% for GGT and the highest specificity was 93.02% for LAP.The sensitivity,specificity and Yoden index of joint detection were higher than those in single test.Conclusion The combined detection of LAP,GGT and CA199 is of great value in the diagnosis of early pancreatic cancer.
