药物涂层球囊治疗下肢动脉硬化闭塞症的疗效观察

摘要 目的:探究采用药物涂层球囊技术治疗下肢动脉硬化闭塞症的治疗效果。方法:选择2016年12月至2018年1月入院的确诊为下肢动脉硬化闭塞症患者共64例,随机分为对照组与研究组,分别采用金属裸支架法与药物涂层球囊法给予治疗,然后比较两组不同方案的治疗效果。结果:两组患者治疗后最小管腔直径以及踝肱指数与治疗前比具有显著差异(P <0. 05);对照组中有9例出现靶血管血运重建(28. 13%),实验组中有1例患者出现靶血管血运重建(3. 12%),差异显著(P <0. 05);研究组明显减少支架的释放或缩短释放支架的长度;两组患者均未见术中死亡、术后30天手术相关性死亡、患病肢体截肢情况;对照组有4例继发血栓(3. 12%),观察组中仅有1例继发血栓(12. 50%),两组数据比较差异显著(P <0. 05)。结论:采用药物涂层球囊法治疗患者下肢动脉硬化闭塞症可以显著改善患者晚期管腔损失,扩大最小管腔直径,提高踝肱指数并且降低靶血管血运重建,减少支架的释放或缩短释放支架的长度,治疗效果显著,值得推广应用。 Objective:To explore the therapeutic effect of drug-coated balloon technique for the treatment of arteriosclerosis obliterans in lower extremities.Methods:A total of 64 patients with definite diagnosis of lower extremity arteriosclerosis obliterans who were admitted to the hospital from December 2016 to January 2018 were randomly divided into a control group and a study group.They were treated with bare metal stents and drug-coated balloons,Then compared the treatment effects of the two groups.Results:The minimum lumen diameter and sputum index after treatment in the two groups were significantly different from those before treatment(P<0.05).In the control group,target vessel revascularization occurred in 9 cases(28.13%),and target vessel revascularization occurred in 1 patient in the experimental group(3.12%),which with significant difference(P<0.05).The study group significantly reduced the release of the stent or shortened the length of the stent;both patients did not have intraoperative death,surgical-related death 30 days after surgery,limb limb amputation;There were 4 cases of secondary thrombosis(3.12%)in the control group and only 1 case of secondary thrombosis(12.50%)in the observation group.There was a significant difference between the two groups(P<0.05).Conclusion:The use of drug-coated balloon therapy for patients with lower extremity arteriosclerosis obliterans could significantly improve late lumen loss,expand the minimum lumen diameter,increase the ankle index,and reduce target vessel revascularization,reducing stent release or shortening the release.The length of the stent was significant and it was worthy of promotion and application.
