摘要
目的:研究三维能量多普勒超声(3D-PDU)联合二维彩色多普勒超声(2D-CDFI)对胎盘植入孕产妇产前诊断效能的影响。方法:选取我院128例胎盘植入孕产妇作为观察组,另选同期健康孕产妇体检者120例为对照组,均采取3D-PDU、2D-CDFI检查,比较两组胎盘血流参数[血流指数(FI)、血管指数(VI)、血管血流综合指数(VFI)],并比较3D-PDU、2D-CDFI检查胎盘植入孕产妇的阳性检出率。结果:观察组胎盘VI、VFI高于对照组,差异有统计学意义(P <0. 05);两组胎盘FI无明显差异(P> 0. 05); 3D-PDU联合2D-CDFI检查胎盘植入孕产妇阳性检出率94. 53%高于单独检查78. 13%、64. 84%,差异有统计学意义(P <0. 05)。结论:3D-PDU联合2D-CDFI检查应用于胎盘植入孕产妇诊断中阳性检出率较高,有助于提高诊断准确率,为临床评估病情、选择合适分娩时间与方式提供依据。
Objective:To investigate the effect of three-dimensional energy Doppler ultrasound(3D-PDU)combined with two-dimensional color Doppler ultrasound(2D-CDFI)on prenatal diagnostic efficacy of pregnant women with placenta accreta.Methods:A total of 128 cases of pregnant women with placenta accreta in our hospital were selected as the observation group,and another 120 cases of medical examiners of healthy pregnant women were selected as the control group at the same time.Both 3D-PDU and 2D-CDFI examinations were taken in two groups.The placental blood flow parameters[blood flow index(FI),vascular index(VI),comprehensive index of blood vessel blood flow(VFI)]of two groups were compared.The positive rate of pregnant women with placenta accreta detected by 3D-PDU and 2D-CDFI was compared.Results:The VI and VFI of the placenta in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).There was insignificant difference between the two groups in placenta FI(P>0.05).The positive rate of pregnant women with placenta accreta detected by 3D-PDU and 2D-CDFI was 94.53%,higher than that of 3D-PDU 78.13%and 2D-CDFI 64.84%,respectively.The difference was statistically significant(P<0.05).Conclusion:The application of 3D-PDU combined with 2D-CDFI in the diagnosis of pregnant women with placenta accreta has high positive detection rate.It is helpful to improve the accuracy of diagnosis,and to provide the basis for the clinical evaluation of the disease and the choice of proper delivery time and delivery mode.
