尿NGAL和血清胱抑素C联合检测对急性肾小球肾炎早期肾损伤诊断价值分析

Diagnostic Value of Combined Detection of Urinary NGAL and Serum Cystatin C in Early Renal Injury of Acute Glomerulonephritis

目的:分析尿中性粒细胞明胶酶相关脂质运载蛋白(尿NGAL)和血清胱抑素C联合检测对急性肾小球肾炎早期肾损伤的诊断价值。方法:选择2017年1月-2018年7月笔者所在医院急性肾小球肾炎患者66例,设为观察组;选择同期60例健康体检者纳入对照组。比较两组患者尿NGAL、血清胱抑素C水平,分析尿NGAL、血清胱抑素C单独检测及联合检测急性肾小球肾炎的灵敏度、特异度。结果:观察组患者尿NGAL、血清胱抑素C水平均明显高于对照组,差异有统计学意义(P<0.05);尿NGAL检验、血清胱抑素C检验及两者联合检查急性肾小管肾炎的特异度、灵敏度均较高,且联合检验的灵敏度明显高于两者单独检验,差异有统计学意义(P<0.05)。结论:尿NGAL与血清胱抑素C联合检测在急性肾小球肾炎早期肾损伤诊断中具有较高的特异度与灵敏度,有利于提高诊断的准确率,降低漏诊误诊率,在急性肾损伤早期诊断中具有较大的参考价值。

Objective:To analyze the diagnostic value of combined detection of urinary neutrophil gelatinase-associated lipocalin(urinary NGAL)and serum cystatin C in early renal injury of acute glomerulonephritis.Method:A total of 66 patients with acute glomerulonephritis in our hospital from January 2017 to July 2018 were enrolled in the observation group,60 healthy subjects were selected as the control group.The urinary NGAL and serum cystatin C levels were compared between the two groups.The sensitivity and specificity of urinary NGAL,serum cystatin C alone and combined detection of acute glomerulonephritis were analyzed.Result:The levels of urinary NGAL and serum cystatin C in the observation group were significantly higher than those in the control group(P<0.05).Urinary NGAL test,serum cystatin C test and the combined detection of acute tubular nephritis had high specificity and sensitivity,and the sensitivity of the joint test were significantly higher than the separate test,the differences were statistically significant(P<0.05).Conclusion:Urine NGAL combined with serum cystatin C detection has high specificity and sensitivity in the diagnosis of early renal injury of acute glomerulonephritis,which is conducive to improve the accuracy of diagnosis and reduce the rate of missed diagnosis and misdiagnosis in the early diagnosis of acute kidney injury.
