摘要
目的:探讨腔内心电图定位PICC导管尖端的临床效果。方法:选择2016年7月-2018年7月150例PICC患者,参照随机数字表分为研究组和对照组,各75例。对照组采用常规PICC置管技术,置管成功后给予胸部正位X线摄片,研究组在对照组基础上实施腔内心电图实时定位,成功后给予胸部正位X线摄片。比较两组一次尖端到位率、异位率和尖端位置(最佳、过浅、过深),记录两组并发症。结果:研究组一次尖端到位率为97.33%,高于对照组的88.00%(P<0.05),研究组一次尖端异位率为2.67%,低于对照组的12.00%(P<0.05)。研究组尖端最佳位置例数(66例)多于对照组(54例),过浅、过深例数少于对照组(P<0.05)。两组渗血、局部感染、导管脱落、机械性静脉炎等比较差异无统计学意义(P>0.05)。结论:腔内心电图定位PICC导管尖端可提高一次到位成功率和最佳位置率,不增加并发症。
Objective:To investigate the effect of intracavitary electrocardiogram on locating PICC catheter tip.Method:A total of 150 tumor chemotherapy patients with ECG-aided tip localization during PICC placement,selected form July 2016 to July 2018,were randomly divided into the study group and the control group,75 cases in each group.In the control group,the conventional PICC tube technique was used,and the chest radiography was given to locate catheter tip.The study group was carried out the real-time location with intracavitary electrogram on the basis of the control group,and the chest X-ray was given to locate catheter tip.The tip first arrival rate,the tip first ectopic rate and tip location(best,shallow and deep)were compared,and the complications(bleeding,local infection,catheter exfoliation,mechanical phlebitis)were recorded in the two groups.Result:The tip first arrival rate in the study group was 97.33%,which was higher than 88.00%in the control group(P<0.05),and the tip first ectopic rate in the study group was 2.67%,which was lower than 12.00%in the control group(P<0.05).The number with the best apex position in the study group was 66 cases,which was more than 54 cases in the control group(P<0.05),and the number with shallow and deep apex position were less than those in the control group(P<0.05).There were no significant difference in bleeding,local infection,catheter ablation and mechanical phlebitis in the two groups(P>0.05).Conclusion:Locating the PICC catheter tip with intracavitary electrocardiogram could increase the tip first arrival rate and the best position rate without increasing complications in tumor chemotherapy patients.
