无创超声心输出量测定对感染性休克患者血流动力学状态评价的意义分析 被引量:3

摘要 目的:分析无创超声心输出量测定对感染性休克患者血流动力学状态评价的意义。方法:选取2016年1月-2018年1月在笔者所在医院进行医治的100例已经确诊为感染性休克的患者作为研究对象,随机分为研究组和对照组,每组50例。研究组进行无创超声心输出量测定指导患者补液治疗和指导用药,对照组进行肺动脉导管热稀释法测定指导患者的补液治疗和指导用药。观察两组患者治疗前后的心输出量(cardiac output,CO)、每搏量(stroke volume,SV)、心指数(cardiac index,CI)的变化;比较两组患者的预后情况。结果:治疗前后研究组和对照组的CO、SV和CI相比差异均无统计学意义(P>0.05),但是治疗后,两组患者的CO、SV和CI都比治疗前有所下降(P<0.05)。两组患者ICU入住时间、机械通气时间和30 d病死率相比差异无统计学意义(P>0.05)。结论:无创超声心输出量测定对感染性休克患者血流动力学状态评价有重要意义。 Objective:To analysis the significance of ultrasonic cardiac output monitor in hemodynamic status of patients with septic shock.Method:From January 2016 to January 2018,100 septic shock patients in our hospital were chosen as study group and control group,50 cases in each group.The patients in the study group were given non-invasive echocardiographic cardiac output measurement to guide the treatment of rehydration and medication,while the control group was given the method of thermal dilution of pulmonary artery catheter to guide the patients’rehydration therapy and drug use.compared the prognosis and the changes of cardiac output,stroke volume and cardiac index between the two groups before and after treatment.Result:There were no significant differences in CO、SV and CI between the study group and the control group before and after treatment(P>0.05),but after treatment,the CO、SV and CI of the two groups were lower than those before treatment(P<0.05).There were no significant differences in ICU occupancy time,mechanical ventilation time and 30 d mortality between the two groups(P>0.05).Conclusion:Noninvasive echocardiographic measurement of cardiac output is of great significance in the evaluation of hemodynamic status in septic shock patients.
