通窍活血汤联合西药治疗椎-基底动脉供血不足性眩晕的临床研究

Clinical Study of Tongqiao Huoxue Decoction Combined with Western Medicine in the Treatment of Vertebrobasilar Insufficiency Vertigo

目的探讨通窍活血汤联合西药长春西汀治疗椎-基底动脉供血不足性眩晕的临床疗效。方法将本院就诊的110例椎-基底动脉供血不足性眩晕患者随机分为治疗组和对照组,其中对照组50例,给予西药长春西汀常规治疗;治疗组60例,给予通窍活血汤联合长春西汀治疗;2组疗程均为4周。比较2组临床总有效率;观察2组治疗前后颈动脉内膜中层厚度(IMT)、欧洲眩晕评价量表(EEV)评分、症状评分、血液流变学、血液动力学及血管内皮功能等指标的变化。结果治疗后,治疗组临床总有效率显著高于对照组(P<0.05);2组IMT、EEV评分、症状评分、血液流变学指标、血流动力学指标及血管内皮功能均显著改善(P<0.05),且治疗组改善程度明显优于对照组(P<0.05)。结论通窍活血汤联合西药治疗椎-基底动脉供血不足性眩晕效果优于单用西药,能够明显改善血管内皮功能,降低血液黏稠度,改善椎-基底动脉供血,值得临床进一步研究与推广。

Objective To investigate the clinical effect of Tongqiao Huoxue decoction combined with vinpocetine in the treatment of vertebrobasilar insufficiency vertigo. Methods A total of 110 patients with vertebrobasilar insufficiency vertigo were randomly divided into treatment group( n =60)and control group( n =50).The control group was given the routine treatment of vinpocetine and the treatment group was treated with Tongqiao Huoxue decoction combined with vinpocetine.The course of treatment in both groups was 4 weeks.The total effective rate of the two groups was compared.The changes of carotid intima-media thickness(IMT),European evaluation of vertigo(EEV)scores,symptom scores,hemorheology,hemodynamics and vascular endothelial function indexes were observed in both groups before and after treatment. Results After treatment,the total effective rate in the treatment group was significantly higher than that in the control group( P <0.05).The IMT,EEV scores,symptom scores,hemorheology,hemodynamics and vascular endothelial function indexes of the two groups were significantly improved( P <0.05),and the improvement of the treatment group was significantly better than that of the control group( P <0.05). Conclusion Tongqiao Huoxue decoction combined with western medicine in the treatment of vertebrobasilar insufficiency vertigo can obviously improve vascular endothelial cell function,reduce blood viscosity and improve vertebrobasilar artery blood supply.It is worthy of further clinical research and promotion.
