摘要
目的探讨超声造影与磁共振增强扫描及弥散加权成像(Diffusion Weighted Imaging,DWI)序列在诊断肝脏复发性微小癌中的应用价值。方法选择2016年1月至2018年3月来我院就诊的26例病灶≤1.0 cm复发微小肝癌患者为研究对象,分析其超声造影及磁共振增强扫描及DWI图像特征。结果病灶超声造影及磁共振增强扫描结果显示,26个扫描结果中快进快出分别为14个、11个,快进慢出为6个、4个,快进不出5个、7个,等增强-退出分别为1个、4个。26个病灶DWI全部为高信号。超声造影与磁共振两种检查方法诊断复发性微小肝癌准确性比较,无显著性差异(P>0.05)。结论超声造影与磁共振增强扫描能敏感地显示微小肝癌内部血流动力学特征,均可作为诊断复发性微小肝癌的影像学检查方法。DWI结合磁共振增强扫描可明显提高磁共振诊断准确性。
Objective To evaluate the diagnostic value of contrast-enhanced ultrasound and magnetic resonance imaging (MR) and diffusion-weighted imaging (DWI) in the diagnosis of recurrent microcarcinoma of the liver. Methods A total of 26 cases of small liver cancer with recurrence less than or greater than 1.0 cm who visited our hospital from January 2016 to March 2018 were selected as the research objects, and their contrast-enhanced ultrasound and mri scanning and DWI image characteristics were analyzed. Results The results of ceus and mri enhanced scan showed that there were 14 and 11 fast in and out, 6 and 4 fast in and out, 5 and 7 fast in and out, and 1 and 4 in and out, respectively. All of the 26 foci of DWI were high signal. There was no significant difference in the accuracy of P>0.05 and MR in the diagnosis of recurrent small hepatocellular carcinoma. There was no significant difference between the two methods. Conclusion The contrast-enhanced ultrasound and MR can sensitively display the internal hemodynamic characteristics of small hepatocellular carcinoma, and can be used as an imaging method for the diagnosis of recurrent small hepatocellular carcinoma. DWI combined with enhanced MR can significantly improve the diagnostic accuracy of MR.
