不同年龄前列腺增生患者下尿路症状的相关性分析

Clinical study of lower urinary tract symptoms in different age patients with benign prostatic hyperplasia

目的分析不同年龄前列腺增生(BPH)患者下尿路症状(LUTS)的临床特征及其对生活质量的影响。方法选取2012年6月-2017年1月西安市第一医院泌尿外科收治的BPH患者506例,对其临床资料进行回顾性分析,内容包括患者年龄、病程、国际前列腺症状(IPSS)评分、性功能问卷表(MSF-4)评分与生活质量(QOL)评分等,对各问卷的数据进行统计分析。结果患者以10岁为一年龄组,506例分成5组,LUTS总体评分为13.58分,可信区间为6.41～24.58分;IPSS评分组间两两比较差异均有统计学意义(F=3. 548 P=0. 008);506例BPH相关的LUTS中,轻度42.24%、中度36. 21%、重度25. 51%;经Spearman分析结果显示,IPSS评分与年龄呈正相关(r=0. 751,P=0.025);与MSF-4评分亦呈正相关(r=0.185,P=0.005),与QOL评分呈负相关(r=-0.158,P=0.032),但相关性较小。结论前列腺增生相关的LUTS严重影响患者性功能及生活质量,且年龄越大,下尿路症状越重。

Objective To analyze the clinical features of different age patients with benign prostatic hyperplasia associated lower urinary tract symptoms,to understand the benign prostatic hyperplasia associated lower urinary tract symptoms on the quality of life. Methods The clinical data of 506 patients with benign prostatic hyperplasia admitted in the hospital from June 2012 to January 2017 were retrospectively reviewed.The contents included the age,course of disease,the international prostate symptom questionnaire (IPSS),sexual function questionnaire (MSF-4) and quality of life score hospitalized(QOL), the data on the questionnaire for statistical analysis. Results The patients were divided into 5 groups according to the age of 10 years, the overall LUTS score was 13.58 and the confidence interval was 6.41~24.58.There was significant difference between every two groups in IPSS score ( F =3.548,P = 0.008).Among 506 patients,mild type was 42.24%,moderate type was 36.21%,severe type was 25.51%.Spearman analysis showed that IPSS was positively correlated with age( r =0.751,P =0.025),slightly positively correlated with MSF-4( r =0.185,P =0.005),negatively correlated with QOL( r =-0.158,P =0.032). Conclusion The symptoms of lower urinary tract associated with benign prostatic hyperplasia affect the quality of life and sexual function of patients,and the more severe the lower urinary tract symptoms,the greater the impact.
