深部肌肉刺激治疗对颈肩痛患者肌肉特性及表面肌电信号的影响 被引量:8

摘要 目的分析深部肌肉刺激治疗对颈肩痛患者肌肉特性及表面肌电信号的影响。方法选取内蒙古医科大学附属医院2014年1月~2017年12月收治的120例颈肩痛患者,按就诊编号随机数表法分为观察组与对照组,每组60例;两组均给予布洛芬缓释片+颈肩牵引治疗,对照组进行假刺激,观察组则联合深部肌肉刺激;对比两组疗效、疼痛情况及肌肉特性、表面肌电信号。结果观察组总有效率为85.00%,与对照组的78.3%比较,差异无统计学意义(P> 0.05),但两组疗效等级差异有统计学意义(P <0.05),且观察组临床治愈病例显著高于对照组(P <0.05);治疗后,对照组F值、D值、S值治疗前后未见明显变化(P> 0.05),观察组振动频率(F)、振幅的对数衰减值(D)、动态硬度(S)均显著下降,且低于对照组(P <0.05);治疗后,两组平均肌电振幅值(AEMG)、积分肌电值(IEMG)、频域指标中位频率(MF)均显著上升,且观察组高于对照组(P <0.05)。结论深部肌肉刺激治疗可改善颈肩痛患者肌肉特性及肌肉局部耐受性,提升肌张力及肌肉收缩性。 Objective To analyze the effect of deep muscle stimulation on muscle characteristics and surface electromyogram signals of patients with neck and shoulder pain.Methods 120 patients with neck and shoulder pain who were admitted to the hospital from January 2014 to December 2017 were randomly divided into observation group and control group according to the order of treatment,60 cases in each group.Both groups were given ibuprofen sustained release tablets and neck and shoulder traction treatment,the observation group was additionally given deep muscle stimulation,the control group was additional given false stimulation.The efficacy,pain,muscle characteristics and surface EMG signals were compared between the two groups.Results There was no significant difference in the total response rate between the observation group and the control group(85.00%vs 78.3%)(P>0.05),but the difference in the curative effect was statistically significant(P<0.05).The number of cured cases in observation group was significantly more than that in the control group(P<0.05).The vibration frequency(F),logarithmic attenuation of amplitude(D)and dynamic hardness(S)in the observation group were decreased significantly after treatment and they were lower than those in the control group(P<0.05).There were no obvious changes of F value,D value or S value in the control group before and after treatment(P>0.05).The average EMG(AEMG),integral EMG(IEMG)and median frequency(MF)of frequency domain indicators in both groups were increased significantly after treatment,and they were higher in observation group than in the control group after treatment(P<0.05).Conclusion Deep muscle stimulation can improve muscle characteristics and local muscle tolerance of patients with neck and shoulder pain,and improve muscle tone and muscle contraction.
