经皮椎体成形术治疗陈旧性骨质疏松性椎体压缩骨折疗效探讨

摘要 目的分析经皮椎体成形术治疗陈旧性骨质疏松性椎体压缩骨折的临床疗效。方法随机选取该院收治的80例陈旧性骨质疏松性椎体压缩骨折患者参与本次实验,入选病例均来自2015年1月—2017年10月,采取随机抽签的方式将其分为实验组与对照组,各40例,分别采取经皮椎体成形术与椎体后凸成形术治疗,对比两组患者的手术时间、手术费用、骨水泥渗漏率、疼痛程度、伤椎前柱和中柱高度恢复率、术后一年伤椎高度丢失情况。结果实验组患者手术时间(28.4±5.6)min较对照组更短(t=6.678,P=0.000),手术费用(24 667.8±1 130.6)元较对照组更少(t=58.264,P=0.000),骨水泥渗漏率(27.5%)较对照组(7.5%)更高(χ2=5.541,P=0.019),术后1年伤椎高度丢失(0.52±0.39)mm较对照组更低(t=11.341,P=0.000);两组患者术后1 d的VAS评分、伤椎前柱和中柱高度恢复率均无明显区别,组间比较差异无统计学意义(P>0.05)。结论经皮椎体成形术在治疗陈旧性骨质疏松性椎体压缩骨折中具有较好的疗效,值得在临床中推广应用。 Objective To analyze the clinical efficacy of percutaneous vertebroplasty in the treatment of old osteoporotic vertebral compression fractures.Methods A total of 80 patients with old osteoporotic vertebral compression fractures admitted to our hospital were enrolled in this experiment.The selected cases were from January 2015 to October 2017.They were randomly divided into experimental group and control group by random lottery,40 cases each,were treated with percutaneous vertebroplasty and kyphoplasty respectively.The operation time,operation cost,cement leakage rate,pain degree,and anterior column of the injured vertebral column were compared between the two groups.The height recovery rate of the middle column and the height loss of the injured vertebrae in one year after operation were observed.Results The operation time of the experimental group (28.4±5.6)min was shorter than that of the control group (t=6.678,P=0.000),and the operation cost (24 667.8±1 130.6)yuan was less than that of the control group (t=58.264,P=0.000).The leakage rate (27.5%) was higher than that of the control group (7.5%)(χ^2=5.541,P=0.019),and the height of the injured vertebrae was lower (0.52±0.39) mm in one year after surgery than in the control group (t=11.341,P=0.000);There was no significant difference in the VAS score,the anterior column of the injured vertebrae and the height recovery rate of the middle column between the two groups the different was statistically significant(P>0.05).Conclusion Percutaneous vertebroplasty has a good curative effect in the treatment of old osteoporotic vertebral compression fractures,and it is worthy of popularization in clinical practice.
