艾滋病患者颈动脉弹性超声评价探讨

Evaluation of Carotid Elastic Ultrasound in AIDS Patients

目的探讨艾滋病患者的颈动脉弹性的超声检查价值。方法随机选取新疆维吾尔自治区第六人民医院在2016年6月—2018年6月间收治的符合研究入选标准的36例艾滋病患者作为研究组,并随机选择36名健康体检人员作为对照组,对比两组人员的颈动脉弹性超声检查情况。结果研究组患者的劲动脉弹性参数中IMT值(0.563±0.092)mm、CC (0.54±0.21)mm2/k Pa、β(13.07±6.12)、PWV (8.46±1.53)m/s;对照组健康体检人员颈动脉弹性各项参数IMT值(0.516±0.143)mm、CC(1.06±0.47)mm2/k Pa、β(8.25±1.46)、PWV(6.73±1.82)m/s。研究组与对照组在颈动脉IMT指数方面比较无明显差异(t=6.46,P>0.05)。而研究组患者的颈动脉CC明显降低,β、PWV均明显上升,差异有统计学意义(t=18.24、18.56、18.91,P<0.05)。研究组患者HIV感染时间与IMT、CC、β、PWV相关性r值分别为0.193、-0.572、0.384、0.419;CD4+细胞计数与IMT、CC、β、PWV相关性分别为0.000、-0.092、0.094、0.047;CD8+细胞计数与IMT、CC、β、PWV相关性分别为0.015、-0.094、0.017、0.024。艾滋病感染时间与颈动脉IMT无相关,与颈动脉CC呈负相关,β、PWV呈正相关趋势。CD4+、CD8+细胞计数与颈动脉IMT、CC、β、PWV均无相关性。结论艾滋病患者的颈动脉弹性功能要比健康人士低,且随着艾滋病患者的感染时间越长其颈动脉弹性状态有越来越差的趋势,艾滋病患者接受颈动脉弹性超声检查具临床应用价值,能够在一定程度上帮助判断患者病情情况。

Objective To investigate the value of ultrasonography of carotid artery elasticity in AIDS patients.Methods 36 AIDS patients admitted to Faculty of the Sixth People's Hospital of Xinjiang Uygur Autonomous Region from June 2016 to June 2018 were randomly selected as the study group,and 36 healthy subjects were randomly selected as the control group with arterial elastic ultrasound examination.Results IMT values (0.563±0.092)mm,CC (0.54±0.21)mm^2/kPa,β(13.07±6.12),PWV (8.46±1.53)m/s in the arterial elastic parameters of the study group;The parameters of carotid elasticity of medical examiners were IMT(0.516±0.143) mm,CC(1.06±0.47) mm2/kPa,β(8.25±1.46),and PWV (6.73±1.82) m/s.There was no significant difference in carotid IMT index between the study group and the control group (t=6.46,P>0.05).The carotid artery CC of the study group was significantly decreased,β and PWV were significantly increased,and the difference was statistically significant (t=18.24,18.56,18.91,P<0.05).The correlation between HIV infection time and IMT,CC,β,and PWV in the study group were 0.193,-0.572,0.384,and 0.419,respectively.The correlation between CD4+ cell count and IMT,CC,β,and PWV were 0.000,-0.092,and 0.094,0.047;CD8+ cell count and IMT,CC,β,PWV correlation were 0.015,-0.094,0.017,0.024.There was no correlation between AIDS infection time and carotid IMT,and negative correlation with carotid CC,β and PWV showed a positive correlation trend.There was no correlation between CD4+,CD8+ cell count and carotid IMT,CC,β,PWV.Conclusion Carotid artery elasticity in AIDS patients is lower than that in healthy people,and the carotid elasticity is worsening with the infection time of AIDS patients.The clinical application value of AIDS patients undergoing carotid elastic ultrasound examination,which helps to determine the patient's condition to a certain extent.
