上海市郊区某镇大肠癌高危人群肿瘤早发现行为现况分析

Analysis of the current status of early detection of tumor in high-risk group of colorectal cancer in a suburban town of Shanghai

目的:分析闵行区华漕镇大肠癌高危人群肿瘤早发现行为现况,为肿瘤早发现社区干预提供依据。方法:采用回顾性分析,统计2017年度华漕镇大肠癌高危人群肿瘤早发现行为数据,比较不同年龄、学历、性别、家庭医生签约状况高危人群早发现行为,分析早发现行为的特点及影响因素。结果:大肠癌高危人群中有肿瘤早发现行为者合计544例(93.63%)。≥60岁者肿瘤早发现行为者占比、结肠镜检查率、大便隐血检查率,免费体检率、自行就诊率、社区转诊率、高危随访筛查率明显优于<60岁者(P<0.05);已签约者结肠镜检查率、免费体检率、社区转诊率明显优于未签约者(P<0.05)。Logistic单因素回归分析显示,在结肠镜检查、大便隐血检查方面:≥60岁、与家庭医生签约对大肠癌早发现行为有较积极影响(P<0.05)。结论:华漕镇大肠癌高危人群肿瘤早发现行为,受≥60岁老人体检及家庭医生签约等影响较大。建议今后重点关注<60岁、未签约家庭医生人群的大肠癌早发现干预工作。

Objective:To analyze the current status of early detection of tumor in high risk group of colorectal cancer in Huacao Town,Minhang District,providing the basis for community intervention in early detection of tumor.Methods:The retrospective analysis was used to count up the early detection data of tumor in high risk group of colorectal cancer in Huacao Town,2017, compared the early detection of high-risk groups with different age,educational background,gender and family doctor signing status,and the characteristics and influencing factors of the early detection were analyzed.Results:There were 544 cases(93.63%) with early detection of tumors in high-risk colorectal cancer.Patients aged 60 and above was significantly superior to those under 60 years old in the proportion of early detection of tumor,colonoscopy rate,fecal OB examination rate,free physical examination rate,self-diagnosis rate,community referral rate and high-risk follow-up screening rate(P<0.05).Those who have signed family doctors are clearly superior to those who have not in colonoscopy rate,free physical examination rate and community referral rate(P<0.05).Logistic single factor regression analysis showed,in terms of colonoscopy and fecal OB examination:60 years old and above and family doctor signing status had a positive impact in the early detection of colorectal cancer(P<0.05).Conclusion: Early detection of tumors in high-risk population of colorectal cancer in Huacao Town is greatly influenced by the physical examination for aged 60 and above and family doctor signing status.It is suggested to focus on the early detection and intervention of colorectal cancer in people under 60 years of age and who are not signed by family doctors in the future.
