功能性鼻内镜鼻窦手术后总鼻道填塞与不填塞的比较观察

The comparative observation for packing or not packing of nasal common meatus after functional endoscopic sinus surgery

目的探讨功能性内镜鼻窦手术(FESS)后总鼻道不填塞的安全性和患者体验感。方法 138例患者分成两组,一组为总鼻道不填塞组(A组)85例,另一组为填塞组(B组)53例,比较两组术后的临床症状、并发症及患者体验感(VAS评分)。结果 A组在安全性方面和B组无明显差异,而在患者体验感方面明显优于B组,两组评分差异有统计学意义(P<0.05)。两组在6个月的随访期内均无明显术腔粘连发生。结论通过术前患者病情轻重程度评估,术中恰当的处理,大多数FESS术后患者可不进行总鼻道填塞,这可明显减轻患者的术后痛苦,节省医疗资源,值得推广应用。

Objective To explore the safety and patients’ feeling without nasal common meatus packing after functional endoscopic sinus surgery (FESS). Methods A total of 138 patients were divided into two groups. One group was 85 patients without nasal common meatus packing (group A), and the other group was 53 patients with packing after FESS. The clinical symptoms, complications and patients’ feeling (VAS scores)were compared between the two groups. Results There was no significant difference in safety between group A and group B. The VAS scores of group A were superior to group B, there was significant difference between the two groups(P<0.05). There was no significant intraoperative adhesion in the two groups during the 6-month follow-up period. Conclusion By preoperative evaluation and appropriate surgical treatment, most patients with FESS could avoid nasal common meatus packing, which can significantly reduce postoperative pain and save medical resources.
