摘要 目的探索64排螺旋CT三维重建技术在胫骨平台骨折修复中的应用价值。方法回顾性收集我院骨科收治的100例胫骨平台骨折患者的临床资料,均接受64排螺旋CT三维重建和X线摄片,整理两种方法的诊断符合率、分型准确率等。结果 CT三维重建对胫骨平台骨折术前诊断的符合率为94.0%,分型准确率为90.0%,X线摄片的诊断符合率为76.0%,分型准确率为71.0%,两种方法对比差异均有统计学意义,P<0.05;术后1周的检查中:X线摄片检出32.0%患者有关节面塌陷情况,平均塌陷程度(1.79±1.02)mm,CT三维重建检出46例患者有关节面塌陷情况,平均塌陷程度(2.69±1.15)mm,组间对比差异有统计学意义,P<0.05。结论 64排螺旋CT三维重建技术有利于术前准确诊断胫骨平台骨折,并进行骨折分型,利于手术医师制定科学合理的手术方案,同时在术后应用有助于临床医师观察手术效果,观察关节面有无塌陷情况,为术后综合治疗、功能锻炼等提供可靠依据,值得推广。 Objective To explore the application value of 64-slice spiral CT three-dimensional reconstruction technology in the repair of tibial plateau fracture. Methods Clinical data of 100 patients with tibial plateau fracture admitted and treated in thedepartment of orthopaedicsof our hospital were retrospectively collected.All of them underwent 64-slice spiral CT three-dimensional reconstruction and X-ray,and the diagnostic coincidence rate and typing accuracy of the two methods were sorted out. Results The coincidence rate of CT three-dimensional reconstruction in the preoperative diagnosis of tibial plateau fractures was 94.0%,and the classification accuracy was 90.0%. The diagnostic coincidence rate of X-ray photography was 76.0%,and the classification accuracy was 71.0%. The difference between the two methods was statistically significant(P< 0.05).During the examination 1 week after surgery,X-ray radiography detected 32.0% of the patients with related nodal surface collapse,with an average degree of collapse(1.79±1.02) mm,and CT three-dimensional reconstruction detected 46 patients with related nodal surface collapse,with an average degree of collapse(2.69±1.15) mm. The difference between the two groups was statistically significant,P< 0.05. Conclusion The 64-slice spiral CT three-dimensional reconstruction technique is helpful for accuratepreoperativediagnosis tibial plateau fracture,and fracture classification,advantage physicians make scientific and reasonable operation scheme,operation and application after surgery can help clinicians to observe surgery effect,observe the articular surface with and without failures,such as comprehensive treatment,postoperative function exercise to provide reliable basis,is worth promoting.
