针刺在体外循环心脏手术中的应用进展

Progress in Application of Acupuncture in Cardiac Surgery with Cardiopulmonary Bypass

体外循环(CPB)心脏手术常引起心肌缺血再灌注损伤,心肌缺血再灌注损伤是导致术后并发症的重要因素。针刺作为中国传统医学的重要部分,常用于心血管疾病的治疗。针刺通过调控血管活性物质释放、心肌细胞凋亡及炎症反应等多种机制,可减轻心肌缺血再灌注损伤,对心肌损伤有预防和保护作用,并且可通过调节心脏交感神经与迷走感神经的活性,影响心血管系统功能。针刺复合麻醉用于CPB心脏手术可减少全身麻醉药物用量、维持血流动力学稳定、缓解心肌缺血再灌注损伤,有效降低围术期风险,提高患者术后康复质量,在CPB心脏手术中具有广阔的应用前景。

Cardiopulmonary bypass(CPB) often causes myocardial ischemia reperfusion injury,which is an important factor resulting in postoperative complications.Acupuncture,an important component of traditional Chinese medicine ,is often used in the treatment of cardiovascular diseases.Acupuncture through a variety of mechanisms,such as regulation of the release of vasoactive substances,myocardial cell apoptosis and inflammation,reduces myocardial ischemia reperfusion injury, prevents and protects myocardial injury.Meanwhile,acupuncture can affect cardiovascular system function by regulation of cardiac sympathetic and vagus nerve activity.Acupuncture combined anesthesia in cardiac surgery with CPB can reduce the amount of general anesthesia drugs,maintain hemodynamic stability,and alleviate myocardial ischemia reperfusion injury,which can effectively reduce perioperative risk factors,improve the postoperative rehabilitation,and has a broad application prospect in cardiac surgery with CPB.
