HBV相关肝纤维化中Klotho蛋白的表达及临床意义

Expression and Clinical Significance of Klotho Protein in HBV-related Liver Fibrosis

目的探索Klotho蛋白在乙肝肝纤维化中的表达以及与肝功和透明质酸(HA)的相关性,为非创伤性肝纤维化模型提供新的监测参数。方法收集正常组77例,轻、中、重度乙肝组各59、51、64例,肝硬化组68例5组入选人群标本血清,采用全自动生化仪、酶联免疫吸附试验(ELISA)、磁微粒化学发光法分别检测各组血清肝功指标、Klotho、HA的蛋白水平。结果与正常组相比,血清中Klotho在轻、中、重度慢性乙肝和乙肝肝硬化中表达均更高,HA蛋白水平在重度乙肝组和肝硬化组表达更高(P<0.05)。相关性分析表明Klotho与AST、ALP、TBIL、DBIL、TBA、以及HA蛋白均呈正相关(P<0.05),与ALB、PA呈负相关(P<0.05),与ALT、GGT、TP无相关性(P>0.05)。结论 Klotho蛋白可能成为肝纤维化过程中一个更为敏感的检测指标,它与HA的正相关关系预示着它们在肝纤维化中可能发挥着相同的作用机制。

Objective To explore the expression of Klotho protein in hepatic fibrosis and its correlation with liver function and hyaluronic acid(HA)so as to provide new monitoring parameters for the model of non-traumatic hepatic fibrosis.Methods The serum samples from the normal control group with 77 cases,the mild hepatitis B group group with 59 cases,the moderate hepatitis B group with 51 cases,the severe hepatitis B group with 64 cases,and the cirrhosis group with 68 cases were collected,and the serum liver function indexes and the protein levels of Klotho and HA were detected by the automatic biochemical instrument,enzyme-linked immunosorbent assay(ELISA)and magnetic particle chemiluminescence,respectively.Results Compared with the normal control group,the expression levels of Klotho were significantly higher in the other four groups and the expression level of HA was significantly higher in the severe hepatitis B group and the cirrhosis group(P<0.05).The correlation study showed that Klotho was positively correlated with AST,ALP,TBIL,DBIL,TBA and HA(P<0.05)and negatively correlated with ALB and PA(P<0.05),and it was not correlated with ALT,GGT and TP(P>0.05).Conclusion Klotho protein may be a more sensitive detection index for liver fibrosis,and its positive correlation with HA indicates that they may play the same role in hepatic fibrosis.
