优质护理干预对膝骨关节炎患者疼痛控制及负性情绪的影响

Effect of high quality nursing intervention on pain control and negative emotions of patients with knee osteoarthritis

目的探讨优质护理干预对膝骨关节炎患者疼痛控制及负性情绪的改善效果。方法将133例膝骨关节炎患者研究对象随机分成对照组67例和干预组66例,对照组给予一般性护理干预,干预组在对照组基础上实施优质护理干预(针对性健康教育、心理疏导支持、饮食行为干预、疼痛护理干预和个性化康复锻炼干预)。出院时分别对两组患者疼痛程度、负性情绪及生活质量进行评价。结果出院时两组患者护理干预评价:(1)疼痛程度评价情况:干预组的视觉模拟评分(VAS)评分为(1.14±0.28)分,明显低于对照组(2.61±0.44)分,差异有统计学意义(P <0.05);(2)负性情绪评价情况:干预组的焦虑自评量表(SAS)和抑郁自评量表(SDS)评分分别为(35.46±3.88)分和(30.17±3.90)分,均明显低于对照组(47.98±5.82)分和(42.61±4.49)分,差异有统计学意义(P <0.05);(3)生活质量评价情况:干预组的生活质量量表中文版(WHOQOL-BREF)综合评分为(62.41±4.86)分,明显高于对照组(54.44±4.09)分,差异有统计学意义(P <0.05)。结论优质护理干预不仅有效地减轻患者的疼痛程度,解除其焦虑、忧郁等负性情绪。还能积极促进患者膝关节功能康复,并显著提高其生活质量,对临床有重要指导意义。

Objective To explore the effect of high quality nursing intervention on pain control and negative emotions of patients with knee osteoarthritis.Methods 133 patients with knee osteoarthritis were selected as research objects and they were randomly divided into control group(67 cases)and the intervention group(66 cases).The control group was given general nursing intervention while the intervention group was given high quality nursing intervention(targeted health education,psychological counseling support,dietary behavior intervention,pain nursing intervention and individualized rehabilitation exercise intervention)at basis of the control group.At discharge,the degree of pain,negative emotions and quality of life of patients in two groups were evaluated.Results Nursing intervention evaluation at discharge of patients in two groups:(1)The evaluation of pain degree:the score of visual analogue scale(VAS)in the intervention group was(1.14±0.28),significantly lower than that in the control group(2.61±0.44).The difference was statistically significant(P<0.05).(2)The evaluation of negative emotions:Self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores in the intervention group were respectively(35.46±3.88)and(30.17±3.90),which were significantly lower than those in the control group(47.98±5.82)and(42.61±4.49).The differences were statistically significant(P<0.05).(3)The evaluation of life of quality:the comprehensive score of Chinese version of WHOQOL-BREF of the intervention group was(62.41±4.86),which was significantly higher than that of the control group(54.44±4.09).The difference was statistically significant(P<0.05).Conclusion High quality nursing intervention can not only effectively reduce the degree of pain of patients and relieve their negative emotions such as anxiety and depression,but also positively promote the rehabilitation of knee function and improve the quality of life,which is of important guiding significance in clinical practice.
